"小醒脑开窍"针刺法治疗后循环缺血性眩晕的效果观察

摘　　要：目的分析"小醒脑开窍"针刺法治疗后循环缺血性眩晕(PCIV)的效果。方法抽取2019年5月至2021年4月郑州煤炭(工业)集团有限责任公司总医院收治的PCIV患者78例,按照随机数字表法分为对照组和研究组,每组39例。对照组采用盐酸氟桂利嗪治疗,研究组在对照组治疗基础上采用"小醒脑开窍"针刺法治疗。比较两组疗效及治疗前后眩晕障碍量表(DHI)评分、日常生活活动能力评分(Barthel指数)、血液流变学水平(血浆黏度、全血低切黏度、全血高切黏度)、椎-基底动脉[左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)]血流速度变化。结果研究组总有效率(97.44%,38/39)高于对照组(76.92%,30/39),P<0.05。治疗后,研究组躯体、情绪、功能评分低于对照组,Barthel指数高于对照组(P均<0.05)。治疗后,研究组血浆黏度、全血低切黏度、全血高切黏度低于对照组(P<0.05)。治疗后,研究组LVA、RVA、BA血流速度高于对照组(P<0.05)。结论"小醒脑开窍"针刺法治疗PCIV患者,能缓解眩晕症状,改善血液流变学和椎-基底动脉血流速度,提升患者日常生活活动能力,治疗效果显著。Objective To analyze the effect of Xiaoxingnao Kaiqiao acupuncture on posterior circulation ischemia vertigo(PCIV).MethodsA total of 78 patients with PCIV treated in General Hospital of Zhengzhou Coal(Industry)Group Co.,Ltd.from May 2019 to April 2021 were selected,and they were divided into control group and study group according to the random number table method,with 39 cases in each group.The control group was treated with flunarizine hydrochloride,and the study group was treated by Xiaoxingnao Kaiqiao acupuncture based on the treatment of the control group.The curative effect,dizziness handicap inventory(DHI)score,activity of daily living score(assessed by Barthel index),hemorheological level(plasma viscosity,whole blood low shear viscosity,whole blood high shear viscosity)and blood flow velocity changes of vertebral basilar artery including left vertebral artery(LVA),right vertebral artery(RVA)and basilar artery(BA)were compared between the two groups before and after treatment.ResultsThe total effective rate of the study group(97.44%,38/39)was higher than that of the control group(76.92%,30/39),P<0.05.After treatment,the physical,emotional and functional scores of the study group were lower than those of the control group,while the Barthel index was higher than that of the control group(all P<0.05).After treatment,the plasma viscosity,whole blood low shear viscosity and whole blood high shear viscosity of the study group were lower than those of the control group(P<0.05).After treatment,the blood flow velocity of LVA,RVA and BA in the study group were higher than those in the control group(P<0.05).ConclusionsXiaoxingnao Kaiqiao acupuncture in the treatment of patients with PCIV can relieve the symptoms of vertigo,improve the hemorheology and vertebral-basilar artery blood flow velocity,and improve the activities of daily living.The therapeutic effect is remarkable.
