摘  要:目的探索高密度脂蛋白(HDL)与心房颤动(房颤)的因果关系。方法本研究使用孟德尔随机化(MR)的方法,利用HDL(暴露)的遗传数据,总样本量为400754例,来自54项研究,共314876个单核苷酸多样性(SNP)。房颤(结局)的遗传数据,总样本量1030836例,其中病例60620例,对照970216例,共有314876个SNP;同时纳入芬兰基因库中房颤数据,总样本量138994例,其中病例22068例,对照116926例,共有16379794个SNP;工具变量从400754例参与者的全基因组关联研究(GWAS)中确定。这增加了纳入人群基因的多样性,使结果更稳定。逆方差加权分析法(IVW)作为主要分析方法。敏感性分析用于检验MR分析结果是否可靠。结果IVW显示,HDL不是房颤的危险因素(OR=0.992,95%CI 0.955~1.031,P=0.689;OR=0.947,95%CI 0.884~1.014,P=0.119),两者间不存在因果关联。MR-Egger回归及Cochran’s Q检验表明,IVW结果较为可靠。结论HDL和房颤之间不存在因果关系。ObjectiveTo explore the causal relationship between High-density lipoprotein and atrial fibrillation(AF).MethodsMendelian randomization(MR)was used in this study,using high-density lipoprotein(exposure)genetic data.The total sample size was 400754 patients from 54 studies with 314876 single nucleotide polymorphisms(SNP).Genetic data for AF(outcome),a total sample size of 1030836 cases,of which 60620 cases and 970216 controls.A total of 314876 SNP data on AF in the Finnish gene bank were included,with a total sample size of 138994 cases,including 22068 cases and 116926 controls,with a total of 16379794 SNP.Instrumental variables were identified from a genome-wide association study(GWAS)of 400754 participants.This increased the genetic diversity of the included population,making the results more stable.Inverse variance weighted(IVW)analysis was used as the main analysis method.Sensitivity analysis was used to test the reliability of MR analysis results.ResultsIVW analysis showed that high-density lipoprotein was not a risk factor for AF(OR=0.992,95%CI 0.955-1.031,P=0.689;OR=0.947,95%CI 0.884-1.014,P=0.119).There was no causal association between them.The MR-Egger intercept detection and Cochran's Q test showed that the IVW analysis results were reliable.ConclusionThere is no causal relationship between HDL and AF.
