摘 要:糖尿病肾病(diabetic nephropathy,DN)是导致终末期肾病的关键因素之一,具有进展快、病情重、并发症多等特点。肾小管间质纤维化(tubulointerstitial fibrosis,TIF)是导致DN进展至终末期肾病的核心环节,可通过多种途径加速DN的进展。目前临床上对于DN-TIF尚无特异性的诊疗措施,而外泌体的发现则为阻止甚至逆转这一过程提供了新的目标和方向。本文详细综述了不同类型的外泌体在DN-TIF中发挥的不同作用,以期为今后阻断DN-TIF提供新的诊疗思路。Diabetic nephropathy(DN)is one of the key factors leading to end-stage renal disease,characterized by rapid progression,severe clinical manifestations,and multiple complications.Tubulointerstitial fibrosis(TIF),as a recognized critical process leading to the progression of DN to end-stage renal disease,accelerates the progression of DN through various pathways.At present,there are no specific diagnostic and therapeutic measures for DN-TIF in clinical practice,and the discovery of exosomes provides new targets and directions for preventing or even reversing this process.This review comprehensively summarizes the diverse roles of different types of exosomes in DN-TIF,aiming to provide novel insights and therapeutic strategies for blocking DN-TIF progression.
