摘 要:目的探讨血清中多种微量元素水平与煤工尘肺患病风险的关系,为煤工尘肺的防治拓展新的思路。方法选择2021年6月—2024年1月在新疆维吾尔自治区第三人民医院进行职业性煤工尘肺诊断的患者,确诊职业性煤工尘肺者纳入病例组,未诊断为职业性煤工尘肺者为对照组。进行问卷调查,电感耦合等离子体质谱法测定血清中微量元素浓度。采用多因素logistic回归分析微量元素水平与煤工尘肺患病风险的关系。结果多因素logistic回归分析显示,血清中硒、锌、锶与煤工尘肺存在关联。血清中微量元素水平从低至高四等分,与硒的Q_(1)组相比,Q_(2)、Q_(3)组与煤工尘肺发病风险降低(OR=0.170,95%CI:0.043~0.678;OR=0.217,95%CI:0.050~0.935);与Q_(1)组相比,Q_(2)组锌浓度水平与煤工尘肺发病风险存在负向关联(OR=0.175,95%CI:0.041~0.755);与锶的Q_(1)组相关,Q_(2)、Q_(3)、Q_(4)组发生煤工尘肺的风险增加(OR=5.614,95%CI:1.452~21.717;OR=9.260,95%CI:2.385~35.956;OR=4.638,95%CI:1.163,18.491)。结论中等浓度水平血清中硒和中等浓度水平锌可能是煤工尘肺患病的保护因素;血清中锶水平升高可能是煤工尘肺患病风险增加的危险因素。Objective To explore the relationship between serum levels of various trace elements and the risk of coal workers'pneumoconiosis,and to expand new ideas for the prevention and treatment of coal workers'pneumoconiosis.Methods Patients with occupational coal workers'pneumoconiosis diagnosed in the Third People's Hospital of Xinjiang Uygur Autonomous Region from June 2021 to January 2024 were selected.Those with confirmed occupational coal workers'pneumoconiosis were included in the case group,and those without diagnosed occupational coal workers'pneumoconiosis were included in the control group.Questionnaires were administered and serum trace element concentrations were determined by inductively coupled plasma mass spectrometry.The relationship between trace element levels and the risk of coal workers'pneumoconiosis was analysed by multifactorial logistic regression.Results The multivariate logistic regression analysis showed that serum selenium,zinc and strontium were associated with coal workers'pneumoconiosis.Serum trace element levels were quadrated from low to high,and groups Q_(2) and Q_(3) were associated with a reduced risk of coal workers'pneumoconiosis compared with group Q_(1) for selenium(OR=0.170,95%CI:0.043,0.678;OR=0.217,95%CI:0.050-0.935);there was a negative correlation between the level of zinc concentration and the risk of coal workers'pneumoconiosis in group Q_(2) compared with group Q_(1)(OR=0.175,95%CI:0.041-0.755);Compared with Q_(1)group of strontium,the risk of coal workers'pneumoconiosis was increased in groups Q_(2),Q_(3),and Q_(4)(OR=5.614,95%CI:1.452-21.717;OR=9.260,95%CI:2.385-35.956;OR=4.638,95%CI:1.163-18.491).Conclusion Serum selenium at moderate concentration levels and serum zinc at moderate concentration levels may be protective factors for the development of coal workers'pneumoconiosis;elevated levels of serum strontium levels may be a risk factor for coal workers'pneumoconiosis.
