摘 要:目的探讨汽车制造企业职业人群高尿酸血症患病现状及影响因素。方法对2023年广州市某大型汽车制造企业2094名(高尿酸血症组1283人,对照组811人)在职职工进行问卷调查、体格检查及实验室检测。结果2094名在职工人的高尿酸血症检出率为61.27%。单因素分析结果显示,高尿酸血症组的血尿酸(serum uric acid,SUA)、BMI、收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)、丙氨酸氨基转移酶(alanin aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)及血肌酐(serum creatinine,sCr)水平均高于对照组(t/Z=-57.88、-12.68、-6.51、-5.24、-7.62、-5.95、-8.92,P<0.01)。多因素logistic回归分析结果显示,超重(OR=2.43,95%CI:1.95~3.02)、肥胖(OR=4.19,95%CI:2.85~6.17)、高血压(OR=1.97,95%CI:1.21~3.23)、ALT异常(OR=2.25,95%CI:1.71~2.95)、sCr异常(OR=2.88,95%CI:2.01~4.12)及食肉量较多(OR=1.65,95%CI:1.09~2.50)为高尿酸血症发病的危险因素,吸烟(OR=0.79,95%CI:0.65~0.96)为高尿酸血症发病的保护因素。结论该人群高尿酸血症检出率较高,超重、肥胖、高血压、ALT异常、sCr异常及食肉量多均会增加高尿酸血症的发病风险。Objective To explore the prevalence and influence factors of hyperuricemia among the occupational workers of automobile manufacturing enterprises.Methods A total of 2094 occupational workers(1283 in the hyperuricemia group and 811 in the control group)from a large automobile manufacturing enterprise in Guangzhou were investigated by questionnaire,physical examination,and laboratory tests in 2023.Results The prevalence rate of hyperuricemia among 2094 employed workers was 61.27%.The results of monofactor analysis showed that the levels of serum uric acid(SUA),body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),alanine aminotransferase(ALT),aspartate aminotransferase(AST),and serum creatinine(s Cr)in the hyperuricemia group were higher than those in the control group(t/z=-57.88,-12.68,-6.51,-5.24,-7.62,-5.95,-8.92,P<0.01).Multivariate logistic regression analysis showed that overweight(OR=2.43,95%CI:1.95-3.02),obesity(OR=4.19,95%CI:2.85-6.17),hypertension(OR=1.97,95%CI:1.21-3.23),higher ALT level(OR=2.25,95%CI:1.71-2.95),higher s Cr level(OR=2.88,95%CI:2.01-4.12),and high intake of meat(OR=1.65,95%CI:1.09-2.50)were the risk factors of hyperuricemia,while smoking(OR=0.79,95%CI:0.65~0.96)was a protective factor of hyperuricemia.Conclusion The prevalence rate in the occupational workers of the automobile manufacturing enterprise was relatively high,and overweight,obesity,hypertension,higher ALT level,higher s Cr level,and eating more meat were associated with a higher risk of hyperuricemia.
