摘  要:目的探讨职业性油雾暴露人群肺功能特征及其影响因素。方法将某院2020年1月至2020年9月职业健康检查的在岗职工300例作为研究对象,根据1∶1匹配原则将职业接触油雾超过1年的150例归为暴露组,暴露组中肺功能异常24例为异常组,其余126例为正常组;无油雾接触史150例为对照组。对比暴露组和对照组的肺功能水平、分析肺功能异常比例。同时采用多因素logistic回归分析职业性油雾暴露人群肺功能异常的可能影响因素。结果暴露组肺功能指标FVC为(84.79±3.60)%、FEV_(1)为(85.12±3.83)%、FEV_(1)/FVC为(85.45±5.72)%,低于对照组(86.14±4.94)%、(86.29±4.71)%、(87.25±3.93)%,差异均有统计学意义(t=2.691、2.361、2.361,P<0.05);肺功能异常比例为16.00%,高于对照组(χ^(2)=5.466,P<0.05);异常组年龄为(44.2±10.6)岁、工作年限为(8.12±1.73)年、IL-1β浓度为(148.86±19.03)pg/mL,高于较正常组(37.8±8.7)岁、(7.16±1.14)年、(101.08±13.99)pg/mL(t=2.633、3.179、14.414,P<0.05)。多因素logistic回归显示,IL-1β为职业性油雾暴露人群肺功能异常的危险因素(OR=1.254,95%CI:1.114~1.411,P<0.01)。结论职业性油雾暴露人群肺功能异常与血清中IL-1β水平有关,应重点关注此类人群,避免出现严重肺部损伤。Objective To investigate the characteristics and influencing factors of lung function in people exposed to occupational oil mist.Methods A total of 300 on-duty workers who underwent occupational health examination in a hospital(January 2020 to September 2020)were selected as the study objects,and 150 of them with occupational oil mist exposure history(occupational exposure to oil mist for more than 1 year)were classified into the exposure group according to the 1∶1 matching principle(24 cases of abnormal lung function in the exposure group were classified into the abnormal group,and the other 126 cases were classified as the normal group).The other 150 cases without oil mist exposure history were classified as control group.The level of lung function was compared between the exposed group and the control group,and the proportion of abnormal lung function was analyzed.At the same time,multivariate Logistic regression was used to analyze the possible influencing factors of lung function in the population exposed to occupational oil mist.Results The lung function indexes FVC(84.79±3.60)%,FEV_(1)(85.12±3.83)%,FEV_(1)/FVC(85.45±5.72)%in exposure group were lower than those in control group(86.14±4.94)%,(86.29±4.71)%,(87.25±3.93)%(t=2.691,2.361,2.361,P<0.05),and the proportion of abnormal lung function(16.00%)in exposure group was higher than that of the control group(χ^(2)=5.466,P<0.05).The age(44.25±10.61)years,working years(8.12±1.73)and IL-1β(148.86±19.03)pg/m L level in workers from abnormal group were higher than that of normal group[(37.85±8.72)years,(7.16±1.14)years,(101.08±13.99)pg/m L](t=2.633,3.179,14.414,P<0.05).Multivariate logistic regression showed that IL-1β(OR=1.254,95%CI:1.114-1.411)was a risk factor for abnormal lung function in occupational oil mist exposed population(P<0.01).Conclusion Abnormal lung function in occupational oil mist exposed population is related to IL-1βlevel.Attention should be paid to this population to avoid serious lung injury.
