基于“瘀毒”理论的泛血管疾病中医病机初探

摘　　要：泛血管疾病是众多危险因素共同作用的结果,包括遗传因素、肥胖、吸烟、糖尿病、高血压、血脂异常等传统危险因素,以及空气污染、高同型半胱氨酸血症、高尿酸血症等新兴危险因素。其中,糖脂代谢异常、高血压与泛血管疾病病理生理机制密切相关,泛血管疾病患者常合并糖尿病、血脂异常和高血压病。血脉瘀滞、瘀毒互结是泛血管疾病的核心病机,瘀毒损伤脉道导致脉络虚损是泛血管疾病的广泛内涵。瘀毒既是泛血管疾病的病变要素,也是导致泛血管疾病发展演变的重要环节。故活血解毒是改善血管内皮功能的重要治法,活血解毒中药能够稳定血管内环境,延缓疾病进展,改善患者预后。其具体作用机制包括抗炎、抗氧化应激等,这为从“瘀毒”论治泛血管疾病提供了有力佐证。Pan-vascular disease is the result of a combination of many risk factors,including genetic factors,obesity,smoking,diabetes mellitus,hypertension,dyslipidemia and other traditional risk factors,as well as air pollution,hyperhomocysteinemia,hyperuricemia and other emerging risk factors.Among them,abnormalities of glucose and lipid metabolism and hypertension are closely related to the pathophysiologic mechanisms of pan-vascular diseases,and patients with pan-vascular diseases often have a combination of diabetes mellitus,dyslipidemia and hypertension.Blood stasis and stasis and Toxin interactions are the core pathogenesis of panvascular diseases,and collateral deficiency induced by pulse channel injury caused by stasis and Toxin is the broad connotation of pan-vascular diseases.Blood stasis and Toxin are both the pathogenetic elements of pan-vascular diseases and the important links leading to the development and evolution of pan-vascular diseases.Therefore,blood circulation promoting and detoxification is the important treatment method to improve the endothelial function of blood vessels,and traditional Chinese medicines functioning in blood circulation promoting and detoxification can stabilize the endovascular environment,slow down the disease progression and improve the prognosis of patients.Their specific mechanisms of action include anti-inflammatory and anti-oxidative stress,which provides strong evidence for the treatment of pan-vascular diseases from the perspective of"stasis and Toxin".
