中医药对雷公藤制剂生殖毒性减毒增效作用研究进展

摘　　要：雷公藤及相关制剂致生殖系统损伤的发生率高,且作用机制复杂,其机制主要涉及损伤生殖细胞、诱导细胞凋亡、氧化应激、抑制血管生成、基因异常表达、脂质代谢紊乱、损伤线粒体功能等,而中药可不同程度减轻雷公藤制剂的生殖毒性,以补肾为本,益肾填精、调补阴阳类中药配合活血、疏肝、利湿等治法,可使肾中阴阳平衡,恢复生殖功能。此外,针刺、艾灸、穴位埋线、炮制等中医疗法亦可有效减轻雷公藤对人体生殖系统的损伤。雷公藤制剂种类繁多,今后的实验研究需进一步对比其厂家、药物规格及剂型,发掘更为安全有效的制剂,以利于临床应用,同时基于基因组学、蛋白质组学等技术开展雷公藤及其制剂的生殖毒性及减毒增效研究,为临床合理用药提供参考。The incidence of reproductive system damage caused by Leigongteng and related preparations is high and the mechanism of action is complex,which mainly involves damage to germ cells,induction of apoptosis,oxidative stress,inhibition of angiogenesis,abnormal gene expression,lipid metabolism disorders,and damage to mitochondrial function.Traditional Chinese medicine can reduce the reproductive toxicity of Leigongteng preparation to different degrees.Taking tonifying the Kidneys as the basis,benefiting the Kidney and filling the Essence,combined with Chinese medicine functioning in tonifying the Yin and Yang,by activating the blood,soothing the Liver and inducing Dampness,the Yin and Yang of the Kidney can be balanced and the reproductive function can be restored.In addition,acupuncture,moxibustion,acupoints buried in threads,drug processing and other Chinese medical treatments can also effectively reduce the damage to the human reproductive system caused by Leigongteng.There are many kinds of Leigongteng preparations,and future experimental studies need to further compare the manufacturers,drug specifications and dosage forms to find safer and more effective preparations for clinical application.Besides,studies on reproduction toxicity and attenuation and synergistic effect of Leigongteng and its preparations based on genomics,proteomics and other technologies should be carried out,so as to provide references for the proper use of clinical medication.
