摘　　要：目的筛选出门诊老年患者用药存在风险的药物,优化其在处方前置审核系统中的老年人用药规则。方法回顾性抽取2023年1至6月门诊处方,根据中国老年人潜在不适当用药判断标准评估老年患者(≥65岁)的潜在用药风险,根据风险高低,优化老年人相关药品审核规则,再次抽取2024年1月门诊处方,评估分析审核规则优化成效。结果通过优化潜在不适当用药品种的老年人用药规则,老年患者潜在不适当用药发生率由1.27%降至1.10%,老年患者疾病状态下潜在不适当用药发生率由0.88%降至0.83%,优化后对于潜在用药风险较高的患者可及时给予医师警示。结论门诊老年患者存在一定的潜在不适当用药风险,结合处方优化药品审核规则后,不仅降低了老年患者的用药风险,而且使处方审核系统更契合临床实际情况,保障了患者的用药安全。Objective To identify the potentially inappropriate medicines to elderly outpatients and optimize the elderly medication rules in the prescription pre-review system.Methods A retrospective analysis was conducted on outpatient prescriptions from January to June in 2023.Based on criteria of potentially inappropriate medications for older adults in China,the potential medication risks for elderly patients(65 years and older)were assessed.On the base of the risk levels,the medication review rules for the elderly were optimized.Outpatient prescriptions from January in 2024 was re-extracted,and the effectiveness of the optimization was evaluated.Results By optimizing the prescription pre-review rules for potentially inappropriate medications,the rate of potentially inappropriate medications in elderly patients was decreased from 1.27%to 1.10%,the rate of potentially inappropriate medications under specific disease conditions in elderly patients was decreased from 0.88%to 0.83%,and warnings are provided to doctors for patients with higher potential medication risks.Conclusion There is a certain risk of potentially inappropriate medications in elderly outpatients.By optimizing the medication review rules based on prescription data,the medication risks for elderly patients are reduced,and the prescription review system better aligns with clinical practice,ensuring the medication safety of patients.
