通窍鼻炎颗粒联合曲安奈德鼻喷雾剂治疗变应性鼻炎的疗效观察及对Th1/Th2失衡的影响

摘　　要：目的:探讨通窍鼻炎颗粒联合曲安奈德鼻喷雾剂治疗变应性鼻炎的临床疗效及对患者辅助性T细胞1(Th1)/辅助性T细胞2(Th2)失衡的影响。方法:选取2021年1月~2023年6月期间于某院治疗的100例变应性鼻炎患者作为研究对象,采用随机数字表法分为对照组和观察组,每组50例。对照组患者给予曲安奈德鼻喷雾剂治疗,观察组患者在对照组治疗基础上加用通窍鼻炎颗粒,两组均持续治疗4周。比较两组中医证候积分、Th1/Th2相关因子[白介素-2(IL-2)、γ干扰素(IFN-γ)、白介素-4(IL-4)、白介素-10(IL-10)]、临床疗效及不良反应发生情况。结果:治疗后,两组患者鼻塞、鼻痒、喷嚏频繁发作、鼻涕症状的中医证候积分均降低(P<0.05),且观察组低于对照组(P<0.05);IL-2、IFN-γ和IL-10水平均升高(P<0.05),且观察组高于对照组(P<0.05);IL-4水平降低(P<0.05),且观察组低于对照组(P<0.05)。观察组患者临床治疗总有效率(94.00%)高于对照组(76.00%,P<0.05)。两组患者不良反应总发生率比较无统计学差异(P>0.05)。结论:通窍鼻炎颗粒联合曲安奈德鼻喷雾剂可提高变应性鼻炎患者的临床疗效,改善中医证候,调节Th1/Th2失衡,且不会增加不良反应的发生风险。Objective:To investigate the clinical efficacy of Tongqiao Biyan granules combined with triamcinolone acetonide nasal spray in the treatment of allergic rhinitis and its effect on T-helper 1(Th1)/T-helper 2(Th2)imbalance.Methods:A total of 100 patients with allergic rhinitis treated in a hospital from January 2021 to June 2023 were selected and assigned to the control group and the observation group by random number table method,with 50 patients in each group.The control group was treated with triamcinolone acetonide nasal spray,and the observation group was treated with Tongqiao Biyan granules in addition to the treatment given in the control group.Both groups were treated for 4 weeks.The traditional Chinese medicine(TCM)syndrome scores,Th1/Th2 related factors[interleukin-2(IL-2),interferon-γ(IFN-γ),interleukin-4(IL-4),interleukin-10(IL-10)],clinical efficacy and adverse reactions were compared between the two groups.Results:After treatment,the TCM syndrome scores for nasal obstruction,nasal itching,frequent sneezing and nasal discharge were decreased in both groups(P<0.05),with the observation group showing lower scores than the control group(P<0.05).The IL-2,IFN-γand IL-10 levels were increased in both groups(P<0.05),with the observation group showing higher levels than the control group(P<0.05).The IL-4 level was decreased in both groups(P<0.05),with the observation group showing lower levels than the control group(P<0.05).The total effective rate in the observation group(94.00%)was higher than that in the control group(76.00%,P<0.05).No statistically significant difference was observed in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Tongqiao Biyan granules combined with triamcinolone acetonide nasal spray can improve the clinical efficacy in patients with allergic rhinitis,improving the TCM syndrome,regulating Th1/Th2 imbalance,without increasing the risk of adverse reactions.
