摘　　要：目的评估新疆生产建设兵团(简称“兵团”)医联体或医共体牵头单位临床实验室全血细胞计数(CBC)检测结果的一致性。方法提取兵团19家医联体或医共体牵头单位临床实验室2023年3月和9月CBC白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(Hb)、血小板计数(PLT)、红细胞比容(HCT)及室内质控日间精密度(CV日间)5个项目数据。收集汇总各家临床实验室3个批号质控品5个项目的测定结果,对数据进行统计分析。结果CBC 5个项目中,WBC室内质控CV日间指标19家实验室均满足行业标准的要求,其余项目12家实验室达到行业标准要求。19家实验室3个批号5个项目检测结果的均值(x)、极差(R)分别为:WBC 3.08×10^(9)/L、0.70×10^(9)/L,13.28×10^(9)/L、2.64×10^(9)/L和4.66×10^(9)/L、0.71×10^(9)/L;RBC 2.74×10^(12)/L、0.13×10^(12)/L,5.72×10^(12)/L、0.40×10^(12)/L和3.94×10^(12)/L、0.20×10^(12)/L;Hb 53.32 g/L、4.00 g/L,108.95 g/L、10.00 g/L和71.74 g/L、5.00 g/L;PLT 99.37×10^(9)/L、54.00×10^(9)/L,240.63×10^(9)/L、60.00×10^(9)/L和204.63×10^(9)/L、92.00×10^(9)/L;HCT 17.86%、3.30%,39.52%、7.90%和24.86%、4.90%,19家实验室RBC和Hb测定结果的一致性较好,其他项目稍逊。Mindray检测系统(14家)与Sysmex检测系统(5家)3个批号5个项目检测结果比较显示,2个检测系统间WBC批号2、Hb批号1和2、PLT批号1和3及HCT测定的3个批号检测结果比较,差异均有统计学意义(均P<0.05)。PLT批号1和3检测结果在2个检测系统间相对偏移在20%以上,HCT批号1和3检测结果在2个检测系统间相对偏移在10%以上。结论兵团医联体或医共体牵头单位临床实验室CBC检测结果总体上有较好的一致性,但仍需进一步加强CBC室内质控的管理工作,且重点关注不同检测系统检测结果的差异,以利于推动检验结果互认相关工作的进程。Objective To evaluate the agreement of complete blood count(CBC)test results across clinical laboratories within the leading units of the Xinjiang Production and Construction Corps(referred to as“XPCC”)medical consortium.Methods Five CBC items including white blood cell count(WBC),red blood cell count(RBC),hemoglobin(Hb),platelet count(PLT),hematocrit(HCT)and within-day precision of internal quality control(CV)data of clinical laboratories of 19 medical alliances or medical alliances leading units in March and September 2023 were extracted.The measurement results for these 5 projects were collected from 3 batches of survey samples across the 19 clinical laboratories,followed by a statistical analysis of the data.Results The internal quality control CV s for all 12 laboratories over a two-month period for CBC and 5 projects met the industry standard requirements.However,only one project,the WBC,fulfilled the industry standard requirements from all laboratories′internal quality control CV s for the two-month period.The mean(x)and the range(R)of the test results for 3 batches and 5 projects from 19 laboratories were as follows WBC 3.08×10^(9)/L,0.70×10^(9)/L,13.28×10^(9)/L,2.64×10^(9)/L and 4.66×10^(9)/L,0.71×10^(9)/L;RBC 2.74×10^(12)/L,0.13×10^(12)/L,5.72×10^(12)/L,0.40×10^(12)/L and 3.94×10^(12)/L,0.20×10^(12)/L;Hb 53.32 g/L,4.00 g/L,108.95 g/L,10.00 g/L and 71.74 g/L,5.00 g/L;PLT 99.37×10^(9)/L,54.00×10^(9)/L,240.63×10^(9)/L,60.00×109/L,and 204.63×10^(9)/L,92.00×10^(9)/L;HCT 17.86%,3.30%,39.52%,7.90%and 24.86%,4.90%.The agreement of RBC and Hb measurement results from 19 laboratories was satisfactory,whereas the agreement of other parameters was somewhat less favorable.A comparison of detection results from 3 batches and 5 projects between the Mindray detection system(14 laboratories)and the Sysmex detection system(5 laboratories)indicated that there were statistically significant differences in the test results of WBC batch 2,Hb batches 1 and 2,PLT batches 1 and 3,and HCT batches 1,2 and
