摘  要:目的探讨miR-224和自噬相关基因-5(ATG5)与早发型子痫前期的相关性。方法选取该院产科2021年12月至2022年10月诊治的单纯妊娠期高血压孕妇35例作为A组,早发型子痫前期孕妇35例作为B组,同时选取正常妊娠孕妇35例作为对照组。采集3组孕妇孕24~28周血清标本,采用反转录荧光定量聚合酶链反应检测miR-224、ATG5水平,比较不同组别血清miR-224、ATG5水平;采用多因素Logistic回归分析发生早发型子痫前期的影响因素;采用Pearson相关分析早发型子痫前期患者miR-224水平与ATG5水平的相关性;采用受试者工作特征(ROC)曲线分析miR-224、ATG5预测早发型子痫前期的价值。结果与对照组比较,A组、B组血清miR-224、ATG5水平升高,差异均有统计学意义(P<0.05);与A组比较,B组血清miR-224、ATG5水平明显升高,差异均有统计学意义(P<0.05)。miR-224、ATG5水平升高是发生早发型子痫前期的危险因素(P<0.05)。Pearson相关分析结果显示,早发型子痫前期患者miR-224水平与ATG5水平呈正相关(r=0.457,P<0.05)。血清miR-224、ATG5单独预测早发型子痫前期的ROC曲线下面积(AUC)分别为0.823、0.754,二者联合预测早发型子痫前期的AUC为0.922,明显大于二者单独预测的AUC(Z=2.122,P<0.05;Z=2.716,P<0.05)。结论血清miR-224和ATG5对早发型子痫前期有一定的预测价值。Objective To explore the predictive value of miR-224 and autophagy related gene-5(ATG5)for early-onset preeclampsia.Methods A total of 35 cases of simple pregnancy induced hypertension treated in the Department of Obstetrics in Hebei Petro China Central Hospital from December 2021 to October 2022 were selected as group A,35 cases of early-onset preeclampsia were selected as group B,and 35 normal pregnancy volunteers were selected as the control group.Collect prenatal serum samples from three groups of parturients,and use reverse transcription fluorescence quantitative polymerase chain reaction to detect the expression of miR-224 and ATG5.The differences in serum miR-224 and ATG5 expression among different groups were compared,multivariate Logistic regression analysis was used to investigate the influencing factors of early-onset preeclampsia.Pearson correlation analysis was used to investigate the correlation between miR-224 level and ATG5 ilevel n patients with early-onset preeclampsia.The value of receiver operating characteristic(ROC)curve was used to analyze miR-224 and ATG5 in predicting the onset of early-onset preeclampsia.Results Compared with the control group,the levels of serum miR-224 and ATG5 increased in group A and group B,the differences were statistically significant(P<0.05).Compared with group A,the levels of serum miR-224 and ATG5 in group B increased significantly,and the differences were statistically significant(P<0.05).Elevated levels of miR-224 and ATG5 were risk factors for early-onset preeclampsia(P<0.05).Pearson correlation analysis found a positive correlation between miR-224 level and ATG5 level in patients with early-onset preeclampsia(r=0.457,P<0.05).The area under the ROC curve(AUC)of serum miR-224 and ATG5 for predicting the early-onset preeclampsia were 0.823 and 0.754 respectively.The AUC of the combined prediction of miR-224 and ATG5 for early-onset preeclampsia was 0.922,which was significantly greater than the AUC predicted by each individual(Z=2.122,P<0.05;Z=2.716,P<0.05).Co
