摘  要:股骨头坏死(ONFH)的保髋治疗仍是目前骨科一个挑战性的难题。近年来,随着保髋技术及新材料的发展,涌现了以髓芯减压(CD)为基础的众多联合保髋术式。该文从ONFH的发病机制、CD及CD联合治疗方案(生物治疗:骨髓间充质干细胞、富含血小板血浆、骨形态发生蛋白;力学支撑:钽棒、非血管化或血管化植骨)应用于ONFH的研究进展进行综述。目前的保髋治疗方案主要应用于股骨头尚未塌陷及塌陷早期的ONFH,股骨头远期存活率仍是一个难点。提高ONFH尚未塌陷及塌陷早期股骨头远期存活率,同时为塌陷中晚期的ONFH提供一个可行的保髋方案是未来研究方向。Hip preservation treatment of osteonecrosis of femoral head(ONFH)is still a challenging problem in orthopedics.In recent years,with the development of hip preservation technology and new materials,many combined hip preservation procedures based on core decompression(CD)have emerged.This article discusses the mechanism of core decompression for ONFH,CD and CD combination therapy(biological therapy:bone marrow mesenchymal stem cells,platelet-rich plasma,bone morphogenetic protein.Mechanical support:tantalum rod,avascular or vascularized bone graft)in the treatment of ONFH are reviewed.At present,hip preservation treatment is mainly used for ONFH with the femoral head not yet collapsed or in the early stage of collapse,and the long-term survival rate of the femoral head is still a challenge.It is a future research direction that improving the long-term survival rate of the femoral head before and in the early stages of ONFH collapse,while providing a feasible hip preservation plan for ONFH in the middle and late stages of collapse,.
