归脾汤对急性非静脉曲张性上消化道出血气不摄血证型的临床治疗效果

摘　　要：目的探究归脾汤对急性非静脉曲张性上消化道出血气不摄血证型的临床治疗效果。方法选择2022年1月—2023年7月常熟市中医院收治的151例上消化道出血患者,根据不同治疗方法分为对照组(n=75)、研究组(n=76)。对照组应用注射用艾普拉唑钠治疗,研究组在对照组基础上结合归脾汤治疗,两组均治疗7 d。比较两组中医证候积分、黑便时间、住院时间、凝血功能。结果治疗7 d后,研究组中医证候积分均低于对照组,差异均有统计学意义(P均<0.05)。研究组黑便时间为(2.96±0.25)d,住院时间为(8.95±0.62)d,均短于对照组的(3.47±0.58)d、(9.37±1.38)d,差异均有统计学意义(t=2.201、2.418,P均<0.05)。治疗7 d后,研究组D-二聚体、凝血酶原时间、活化部分凝血活酶时间、纤维蛋白原水平均优于对照组,差异均有统计学意义(P均<0.05)。结论急性非静脉曲张性上消化道出血气不摄血证型在常规药物治疗基础上应用归脾汤,可改善临床症状,有助于提高凝血功能,促进患者康复,缩短住院时间。Objective To explore the clinical therapeutic effect of Guipi Decoction on acute non-variceal upper gastrointestinal bleeding with qi failing to control blood syndrome.Methods A total of 151 patients with upper gastrointestinal bleeding admitted to Changshu Traditional Chinese Medicine Hospital from January 2022 to July 2023 were selected and divided into control group(n=75)and study group(n=76)according to different treatment methods.The control group was treated with ilaprazole sodium for injection,and the study group was treated with Guipi Decoction on the basis of the control group.Both groups were treated for 7 days.The traditional Chinese medicine syndrome scores,melena time,hospitalization time and coagulation function of the two groups were compared.Results After 7 days of treatment,the scores of traditional Chinese medicine syndrome scores in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The time of melena in the study group was(2.96±0.25)d,and the hospitalization time was(8.95±0.62)d,which were shorter than(3.47±0.58)d and(9.37±1.38)d in the control group,the differences were statistically significant(t=2.201,2.418,both P<0.05).After 7 days treatment,the levels of D-dimer,prothrombin time,activated partial thromboplastin time and fibrinogen in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion The application of Guipi Decoction on the basis of conventional drug for acute non-variceal upper gastrointestinal bleeding with qi failing to control blood syndrome can improve clinical symptoms,help improve coagulation function,promote patient recovery,and shorten hospital stay.
