补中益气汤加减治疗急诊重症合并Ⅰ型呼吸衰竭的效果

摘　　要：目的分析急诊重症合并Ⅰ型呼吸衰竭患者运用补中益气汤加减治疗的效果。方法回顾性选取2021年1月—2023年12月聊城市东昌府区中医院收治的75例急诊重症合并Ⅰ型呼吸衰竭患者的临床资料,根据治疗方式的不同分为对照组(n=37,传统治疗)、观察组(n=38,在传统治疗基础上联合补中益气汤加减治疗),对比组间治疗总有效率、临床指标、血气指标。结果观察组治疗总有效率为97.37%(37/38),相较于对照组的81.08%(30/37)更高,差异有统计学意义(χ^(2)=5.442,P<0.05)。与对照组相比,观察组呼吸困难改善时间、纳呆乏力改善时间、无创呼吸机使用时间、住院时间均更短,观察组治疗后心率更低,血氧饱和度、动脉血氧分压均更高,差异均有统计学意义(P均<0.05)。组间治疗后呼吸频率对比,差异无统计学意义(P>0.05)。结论补中益气汤加减治疗急诊重症合并Ⅰ型呼吸衰竭,能改善预后,缩短症状消退时间,提高临床疗效。Objective To analyze the therapeutic effect of Buzhong Yiqi Decoction on severe emergency patients complicated with type I respiratory failure.Methods The clinical data of 75 patients with severe emergency complicated with type I respiratory failure admitted to Dongchangfu District Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were retrospectively selected.According to different treatment methods,they were divided into control group(n=37,traditional treatment)and observation group(n=38,combined with Buzhong Yiqi Decoction on the basis of traditional treatment).The total effective rate of treatment,clinical index and blood gas index were compared between the two groups.Results The total effective rate of treatment in the observation group was 97.37%(37/38),which was higher than 81.08%(30/37)compared with the control group,the difference was statistically significant(χ^(2)=5.442,P<0.05).Compared with the control group,the improvement time of dyspnea,fatigue,noninvasive ventilator use time and hospital stay in the observation group were shorter,after treatment,the heart rate of the observation group was lower,the blood oxygen saturation and arterial blood oxygen partial pressure were higher,and the differences were statistically significant(all P<0.05).There was no significant difference in respiratory rate between the two groups after treatment(P>0.05).Conclusion The application of Buzhong Yiqi Decoction in patients with severe emergency combined with typeⅠrespiratory failure can quickly improve the prognosis,shorten the time of symptom regression,and improve the clinical efficacy.
