腹腔镜下胆总管探查术联合腹腔镜胆囊切除术治疗胆囊结石并发胆总管结石的应用价值

摘　　要：目的探究腹腔镜胆囊切除术联合腹腔镜下胆总管探查术治疗胆囊结石并发胆总管结石的价值。方法以手术方法不同,将2021年7月—2023年12月于扬州市第二人民医院接受治疗的108例胆囊结石合并胆总管结石患者分为对照组(54例,通过开腹胆囊切除胆总管探查术治疗)及观察组(54例,通过LCBDE联合LC治疗)。比较两组临床指标、临床疗效及并发症发生率。结果观察组治疗总有效率更高[96.30%(52/54)vs 83.33%(45/54)],差异有统计学意义(χ^(2)=4.960,P<0.05)。观察组术中出血量、术后通气时间、手术时间、住院时间、并发症发生率均更少,差异均有统计学意义(P均<0.05)。结论胆囊结石合并胆总管结石治疗中采用腹腔镜胆囊切除术联合腹腔镜下胆总管探查术的疗效显著,临床指标明显改善,并发症发生风险较低。Objective To explore the value of laparoscopic cholecystectomy combined with laparoscopic common bile duct exploration in the treatment of cholecystolithiasis complicated with common bile duct stones.Methods According to different surgical methods,108 patients with cholecystolithiasis complicated with common bile duct stones who were treated in Yangzhou Second People's Hospital from July 2021 to December 2023 were divided into control group(54 cases,treated by open cholecystectomy and common bile duct exploration)and observation group(54 cases,treated by LCBDE combined with LC).The clinical indicators,clinical efficacy and incidence of complications of the two groups were compared.Results The total effective rate of the observation group was higher[96.30%(52/54)vs 83.33%(45/54)],the difference was statistically significant(χ^(2)=4.960,P<0.05).The intraoperative blood loss,postoperative ventilation time,operation time,hospitalization time and complication rate in the observation group were less than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion The efficacy of laparoscopic cholecystectomy combined with laparoscopic common bile duct exploration in the treatment of cholecystolithiasis and choledocholithiasis is significant,the clinical indicators are significantly improved,and the risk of complications is low.
