摘 要:目的分析在心力衰竭检查中结合超声心动图参数、血清脑钠肽(B-type natriuretic peptide,BNP)诊断的效果。方法回顾性选取2022年5月—2023年5月济宁市兖州区人民医院收治的869例心力衰竭患者和131例健康体检人员的临床资料,所有研究对象均接受了超声心动图检查和血清BNP检测。对比两组超声心动图参数与BNP水平,绘制受试者操作特征(receiver operating characteristic,ROC)曲线分析超声心动图参数、血清BNP单独及联合诊断心力衰竭的效能。结果健康组BNP、左心室收缩末期容积水平分别为(87.56±10.26)pg/mL、(32.62±5.52)mL,均低于心力衰竭组的(426.14±25.67)pg/mL、(86.24±8.50)mL,差异均有统计学意义(t=149.126、69.994,P均<0.05)。健康组左心室射血分数、短轴缩短率水平均高于心力衰竭组,差异均有统计学意义(P均<0.05)。ROC结果显示:BNP与超声心动图参数的联合诊断效能高于BNP和超声心动图各参数单独诊断的效能。结论在心力衰竭检查中BNP与超声心动图参数的联合诊断效能高于各指标单独诊断的效能,能够提供更全面和准确的疾病信息。Objective To analyze the effect of echocardiography parameters and serum B-type natriuretic peptide(BNP)in the diagnosis of heart failure.Methods The clinical data of 869 patients with heart failure and 131 healthy subjects admitted to Jining Yanzhou District People's Hospital from May 2022 to May 2023 were retrospectively selected.All subjects underwent echocardiography and serum BNP detection.The echocardiographic parameters and BNP levels were compared between the two groups,and the receiver operating characteristic(ROC)curve was drawn to analyze the efficacy of echocardiographic parameters,serum BNP alone and in combination in the diagnosis of heart failure.Results The levels of BNP and left ventricular end-systolic volume in the healthy group were(87.56±10.26)pg/mL and(32.62±5.52)mL,respectively,which were lower than(426.14±25.67)pg/mL and(86.24±8.50)mL in the heart failure group,and the differences were statistically significant(t=149.126,69.994,both P<0.05).The levels of left ventricular ejection fraction and fractional shortening in the healthy group were higher than those in the heart failure group,and the differences were statistically significant(both P<0.05).ROC results showed that the combined diagnostic efficacy of BNP and echocardiographic parameters was higher than that of BNP and echocardiographic parameters alone.Conclusion The combined diagnostic efficacy of BNP and echocardiographic parameters in heart failure examination is higher than that of the two alone,which can provide more comprehensive and accurate disease information.
