全身振动疗法应用于痉挛型脑瘫患儿的效果及对其坐位及整体运动平衡能力的改善探讨

摘  要:目的探讨痉挛型脑瘫患儿接受全身振动疗法(whole body vibration,WBV)的临床效果,分析对其坐位及整体运动平衡能力的改善。方法选取2022年6月—2023年12月安徽医科大学第一附属医院接收的100例痉挛型脑瘫患儿,按不同治疗方案分为对照组与观察组,各50例,对照组行常规康复训练,观察组在对照组基础上增加WBV疗法。比较两组粗大功能、10 m步行时间、平衡能力、坐立位转换能力、竖脊肌表面平均肌电值。结果治疗后,观察组的粗大运动功能测试量表中的D、E区评分分别为(28.81±5.38)分、(18.55±6.16)分,均高于对照组的(25.39±5.56)分、(15.27±5.03)分,差异均有统计学意义(t=2.204、2.916,P均<0.05)。治疗后,观察组的10 m步行时间及5次坐立试验时间均短于对照组,儿童平衡量表评分、左、右侧竖脊肌肌电值均高于对照组,差异均有统计学意义(P均<0.05)。结论痉挛型脑瘫患儿采用WBV治疗可减轻肌肉痉挛,快速恢复核心肌群及下肢肌力,增强核心稳定性,显著提升其坐位及整体运动平衡能力。Objective To explore the clinical effect of whole body vibration(WBV)on children with spastic cerebral palsy,and to analyze the improvement of their sitting position and overall motor balance ability.Methods A total of 100 children with spastic cerebral palsy admitted to the First Affiliated Hospital of Anhui Medical University from June 2022 to December 2023 were selected and divided into control group and observation group according to different treatment schemes,with 50 cases in each group.The control group received routine rehabilitation training,and the observation group added WBV therapy on the basis of the control group.The gross function,10-meter walking test,balance ability,sit-to-stand conversion ability and averaged electromyography value of erector spinae surface were compared between the two groups.Results After treatment,the scores of D and E areas in the Gross Motor Function Measure-88 Items of the observation group were(28.81±5.38)points and(18.55±6.16)points,respectively,which were higher than(25.39±5.56)points and(15.27±5.03)points of the control group,the differences were statistically significant(t=2.204,2.916,both P<0.05).After treatment,the 10-meter walking test time and five repetition sit to stand test time of the observation group were shorter than those of the control group,and the score of the Pediatric Balance Scale,averaged electromyography values of the left and right erector spinae in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion WBV treatment for children with spastic cerebral palsy can reduce muscle spasm,quickly restore core muscle group and lower limb muscle strength,enhance core stability,and significantly improve their sitting and overall motor balance.
