摘  要:目的分析稽留流产患者绒毛组织中纤粘连蛋白可变性剪切片段(FN-EDA)表达量与微血管密度的相关性。方法纳入深圳市龙华区妇幼保健院2022年1月至2023年8月收治的稽留流产患者46例为研究组,随机选取同期人工流产患者46例为对照组。采集研究对象的绒毛组织标本,检测FN-EDA表达量及微血管密度;比较两组研究对象的FN-EDA表达量与微血管密度的差异;分析FN-EDA表达量与微血管密度的相关性;采用受试者工作特征曲线分析FN-EDA表达量预测稽留流产的价值。结果研究组与对照组绒毛组织FN-EDA全部为阳性,其中研究组(+++)占比(8.70%)低于对照组(82.61%),差异有统计学意义(χ^(2)=64.061,P<0.001);研究组和对照组FN-EDA光密度值分别为0.106±0.044、0.224±0.079,微血管密度分别为8.74±0.801、13.30±2.346,差异有统计学意义(t值分别为-8.909、-17.960,P<0.001);FN-EDA光密度值与微血管密度呈正相关(r=0.474,P<0.001);FN-EDA光密度值预测稽留流产曲线下面积(AUC)为0.934,灵敏度为0.804,特异度为0.935,最佳临界值为0.146;微血管密度预测稽留流产AUC为0.972,灵敏度为0.913,特异为0.957,最佳临界值9.218。结论稽留流产者绒毛组织FN-EDA阳性表达量低于人工流产者,FN-EDA光密度值与微血管密度正相关,FN-EDA表达量对预测稽留流产有较高价值。Objective To analyze the correlation between the expression of fibronectin variable splice fragment(FN-EDA)and microvascular density in the villous tissues of patients with missed miscarriage.Methods A total of 46 patients with missed miscarriage admitted to Shenzhen Longhua District Maternal and Child Health Hospital from January 2022 to August 2023 were enrolled as the study group,and 46 patients with induced abortion during the same period were randomly selected as the control group.The villous tissue samples of the study subjects were collected to detect the expression of FN-EDA and the density of microvessels.The differences in FN-EDA expression and microvascular density between the two groups were compared.The correlation between FN-EDA expression and microvessel density was analyzed.The receiver operating characteristic curve was used to analyze the value of FN-EDA expression in predicting missed miscarriage.Results The FN-EDA of villus tissue in the study group and the control group was all positive,and the proportion of villus tissue in the(+++)study group(8.70%)was lower than that in the control group(82.61%),and the difference was statistically significant(χ^(2)=64.061,P<0.001).The optical density of FN-EDA in the study group and the control group were 0.106±0.044 and 0.224±0.079,and the microvessel density were 8.74±0.801 and 13.30±2.346,respectively,with statistically significant differences(t values were-8.909 and-17.960,P<0.001,respectively).The FN-EDA optical density value is positively correlated with microvascular density(r=0.474,P<0.001).The area under the curve(AUC)of FN-EDA optical density value for predicting the area under the curve missed miscarriage was 0.934,the sensitivity was 0.804,the specificity was 0.935,and the optimal critical value was 0.146.The AUC of microvessel density in predicting missed miscarriage was 0.972,the sensitivity was 0.913,the specificity was 0.957,and the optimal cutoff value was 9.218.Conclusion The positive expression level of FN-EDA in the chorionic tiss
