摘 要:目的基于大数据体检资料构建糖尿病前期发病风险简易预测模型,以期利用该模型对体检人群进行糖尿病发病风险预测。方法选取2021—2024年在宁夏医科大学某附属健康管理中心体检的空腹血糖≤7.0 mmol/L人群,收集其人口学特征、一般体格检查、生化指标和尿常规指标等基础资料,采用非条件二分类Logistic回归分析探讨糖尿病前期发病的影响因素。结果76618例空腹血糖≤7.0 mmol/L体检人群中,正常糖耐量72930例(95.19%),糖尿病前期3688例(4.81%)。正常糖耐量和糖尿病前期人群的年龄、性别、婚姻状况、体质指数(BMI)、舒张压、收缩压、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、天门冬氨酸氨基转移酶/丙氨酸氨基转移酶(AST/ALT)、尿素、肌酐(Cr)、尿酸(UA)水平及尿葡萄糖、尿蛋白质比较,差异具有统计学意义(P<0.01)。非条件二分类Logistic回归分析结果显示,年龄、BMI、血压、TG、尿素、UA是体检人群中正常糖耐量向糖尿病前期自然转化的独立危险因素(P<0.05);未婚、HDL-C、AST/ALT是体检人群中正常糖耐量向糖尿病前期自然转化的保护因素(P<0.05)。结论年龄、婚姻状况、BMI、血压、TG、尿素、UA、HDL-C水平以及AST/ALT是体检人群中正常糖耐量向糖尿病前期自然转化的影响因素。Objective To construct a simple prediction model of prediabetes risk based on big data physical examination data,in order to use this model to predict the risk of diabetes in physical examination population.Methods The population with fasting blood glucose≤7.0 mmol/L who underwent physical examination in a health management center affiliated to Ningxia Medical University from 2021 to 2024 were selected.The basic data such as demographic characteristics,general physical examination,biochemical indexes and urine routine indexes were collected.Unconditional binary Logistic regression was used to analyze the influencing factors of prediabetes.Results Among the 76618 subjects with fasting blood glucose≤7.0 mmol/L,72930(95.19%)had normal glucose tolerance and 3688(4.81%)had prediabetes.There were significant differences in age,gender,marital status,body mass index(BMI),diastolic blood pressure,systolic blood pressure,total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),aspartate aminoxansferase/alanine aminotransferase(AST/ALT),urea,creatinine(Cr),uric acid(UA),urinary glucose and urinary protein between normal glucose tolerance and prediabetes(P<0.01).The results of unconditional binary Logistic regression analysis showed that age,BMI,blood pressure,TG,urea and UA were independent risk factors for the natural transformation of normal glucose tolerance to prediabetes in the physical examination population(P<0.05);unmarried,HDL-C and AST/ALT were protective factors for the natural transformation of normal glucose tolerance to prediabetes in the physical examination population(P<0.05).Conclusion Age,marital status,BMI,blood pressure,TG,urea,UA,HDL-C levels and AST/ALT are the influencing factors for the natural transformation of normal glucose tolerance to prediabetes in the physical examination population.
