摘　　要：目的探讨右美托咪定复合丙美卡因表面麻醉在老年白内障超声乳化术中的镇痛效果及对应激反应的影响。方法选取2020年1月至2022年1月80例老年白内障患者为研究对象,以随机数字表法将其分为对照组(n=40,采用丙美卡因表面麻醉)和观察组(n=40,采用右美托咪定复合丙美卡因表面麻醉)。比较两组的麻醉效果。结果白内障摘除(T_(2))、术毕(T_(3))、术后30 min(T_(4)),观察组的Ramsay镇静评分法(RSS)评分高于对照组(P<0.05)。T_(2)、T_(3)、T_(4),观察组的疼痛视觉模拟量表(VAS)评分低于对照组(P<0.05)。T_(3)、T_(4),观察组的简易智能状态检查量表(MMSE)高于对照组(P<0.05)。术中15 min、术后5 min,观察组的丙二醛(MDA)水平低于对照组,超氧化物歧化酶(SOD)水平高于对照组(P<0.05)。结论右美托咪定复合丙美卡因表面麻醉在老年白内障超声乳化术中的镇痛效果显著,可减轻应激反应,值得推广。Objective To investigate the analgesic effect of dexmedetomidine combined with proparacaine surface anesthesia in elderly cataract phacoemulsification and its influence on stress response.Methods A total of 80 elderly cataract patients from January 2020 to January 2022 were selected as the research objects.The patients were divided into control group(n=40,using proparacaine surface anesthesia)and observation group(n=40,using dexmedetomidine combined with proparacaine surface anesthesia)by random number table method.The anesthetic effects of the two groups were compared.Results At the time of cataract extraction(T_(2)),the end of operation(T_(3))and 30 min after operation(T_(4)),the Ramsay Sedation Score(RSS)in the observation group was higher than that in the control group(P<0.05).At T_(2),T_(3)and T_(4),the Visual Analogue Scale(VAS)score of the observation group was lower than that of the control group(P<0.05).At T_(3)and T_(4),the Mini-Mental State Examination(MMSE)in the observation group was higher than that in the control group(P<0.05).At 15 min during operation and 5 min after operation,the level of malondialdehyde(MDA)in the observation group was lower than that in the control group,and the level of superoxide dismutase(SOD)was higher than that in the control group(P<0.05).Conclusion The dexmedetomidine combined with proparacaine surface anesthesia in elderly cataract phacoemulsification has a significant analgesic effect,which can reduce the stress response and is worthy of promotion.
