摘  要:目的探讨超声对Graves病孕妇治疗过程中胎儿甲状腺异常早期诊断和监测的价值。方法选取2022年7月至2024年4月安徽医科大学第一附属医院诊断的33例Graves病孕妇作为研究组,同期选取54名甲状腺功能正常孕妇作为对照组。两组均于孕20~24周及孕28~34周进行胎儿超声检查,观察胎儿甲状腺大小及血流信号。对发现的疑似异常病例实施临床干预,并重复进行超声监测。结果治疗前,两组的胎儿甲状腺体积无显著差异(P>0.05);治疗后,两组的胎儿甲状腺体积大于治疗前(P<0.05),但组间无显著差异(P>0.05)。超声检查提示8例胎儿甲状腺异常,包括2例甲状腺肿大伴血流信号丰富和6例甲状腺血流信号丰富;经治疗后,甲状腺血流信号减少,与脐带血检测结果相一致。结论超声对Graves病孕妇治疗过程中胎儿甲状腺异常的早期诊断和监测具有一定价值;通过给予孕妇药物干预有助于改善胎儿甲状腺功能。Objective To explore the value of ultrasound in the early diagnosis and monitoring of fetal thyroid abnormalities during the treatment of pregnant women with Graves disease.Methods From July 2022 to April 2024,33 pregnant women with Graves disease diagnosed in the First Affiliated Hospital of Anhui Medical University were selected as study group,and 54 pregnant women with normal thyroid function were selected as control group.Fetal ultrasound examination was performed in both groups at 20-24 weeks and 28-34 weeks of gestation,and the fetal thyroid size and blood flow signals were observed.Clinical intervention was performed on the suspected abnormal cases found,and repeated ultrasound monitoring was performed.Results Before treatment,there was no significant difference in fetal thyroid volume between the two groups(P>0.05);after treatment,the fetal thyroid volume of the two groups was larger than that before treatment(P<0.05),however,there was no significant difference between the two groups(P>0.05).Ultrasound examination showed 8 cases of fetal thyroid abnormalities,including 2 cases of thyroid goiter with rich blood flow signal and 6 cases of thyroid blood flow signal;after treatment,the thyroid blood flow signal decreased,which was consistent with the results of umbilical cord blood detection.Conclusion Ultrasound has a certain value in the early diagnosis and monitoring of fetal thyroid abnormalities during the treatment of pregnant women with Graves disease;providing medication intervention to pregnant women can help to improve fetal thyroid function.
