基于个体化的综合护理对老年肺部感染患者康复进程、并发症及呼吸功能的影响

摘　　要：目的探究基于个体化的综合护理在老年肺部感染患者中的应用效果。方法选取78例老年肺部感染患者,以随机数字表法将其分为对照组、观察组,各39例。对照组采用常规护理,观察组在对照组基础上实施基于个体化的综合护理。比较两组的干预效果。结果观察组的自主排痰优良率高于对照组(P<0.05)。观察组的体温恢复时间、湿啰音消失时间、咳痰消失时间及住院时间短于对照组(P<0.05)。干预后,观察组的最大通气量(MVV)、最大呼气中段流量(MMEF)、第1秒用力呼气容积/用力肺活量(FEV_(1)/FVC)高于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。干预后,观察组的匹兹堡睡眠质量指数(PSQI)评分低于对照组,自我护理能力测量量表(ESCA)评分高于对照组(P<0.05)。结论基于个体化的综合护理能够提高老年肺部感染患者的自主咳痰能力和自我护理能力,改善其呼吸功能和睡眠质量,降低并发症发生率,并缩短康复时间,值得推广。Objective To explore the application effect of comprehensive nursing based on individualization in elderly patients with pulmonary infection.Methods A total of 78 elderly patients with pulmonary infection were selected and divided into control group and observation group according to the random number table method,with 39 cases in each group.The control group was given routine nursing,and the observation group was given comprehensive nursing based on individualization on the basis of the control group.The intervention effects of the two groups were compared.Results The excellent and good rate of spontaneous expectoration in the observation group was higher than that in the control group(P<0.05).The body temperature recovery time,moist rale disappearance time,expectoration disappearance time and hospitalization time of the observation group were shorter than those of the control group(P<0.05).After intervention,the maximal voluntary ventilation(MVV),maximal mid-expiratory flow(MMEF),the forced expiratory volume in one second/forced vital capacity(FEV_(1)/FVC)in the observation group were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).After intervention,the Pittsburgh Sleep Quality Index(PSQI)score of the observation group was lower than that of the control group,and the Exercise of Self-Care Agency Scency Scale(ESCA)score was higher than that of the control group(P<0.05).Conclusion Comprehensive nursing based on individualization can improve the ability of spontaneous expectoration and self-care ability of elderly patients with pulmonary infection,elevate their respiratory function and sleep quality,reduce the incidence of complications,and shorten the recovery time,which is worthy of promotion.
