摘　　要：目的分析无隙衔接助产护理在初产妇中的应用效果及对分娩自我效能及分娩情况的影响。方法选择2021年5月至2022年10月在本院建档产检并分娩的100名初产妇作为研究对象,采用随机数字表法将其分为常规组和干预组,各50名。常规组围产期给予常规护理干预,干预组在常规组基础上给予无隙衔接助产护理干预。比较两组的分娩自我效能、产程时间及分娩结局。结果干预前,两组的自我效能、结果期望评分比较,差异无统计学意义(P>0.05);干预后,干预组的自我效能、结果期望评分高于常规组,差异具有统计学意义(P<0.05)。干预组的第一产程、第二产程、第三产程及总产程时间短于常规组,差异具有统计学意义(P<0.05)。干预组的阴道分娩率高于常规组,产后24 h出血量少于常规组,新生儿1 min Apgar评分高于常规组,差异具有统计学意义(P<0.05)。结论无隙衔接助产护理可提升初产妇的分娩自我效能感,缩短产程,改善分娩结局,值得临床进一步推广和应用。Objective To analyze the application effect of seamless midwifery nursing in primipara and its influences on delivery self-efficacy and delivery situation.Methods A total of 100 primiparas who were filed for prenatal examination and delivered in our hospital from May 2021 to October 2022 were selected as the research objects.The primiparas were divided into routine group and intervention group by random number table method,with 50 cases in each group.The routine group was given routine nursing intervention during perinatal period,and the intervention group was given seamless midwifery nursing intervention on the basis of the routine group.The delivery self-efficacy,labor time and delivery outcome were compared between the two groups.Results Before intervention,there were no significant differences in self-efficacy and outcome expectation scores between the two groups(P>0.05);after intervention,the scores of self-efficacy and outcome expectation in the intervention group were higher than those in the routine group,and the differences were statistically significant(P<0.05).The time of the first stage of labor,the second stage of labor,the third stage of labor and the total stage of labor in the intervention group were shorter than those in the routine group,and the differences were statistically significant(P<0.05).The vaginal delivery rate of the intervention group was higher than that of the routine group,the amount of bleeding at 24 h after delivery was less than that of the routine group,and the neonatal 1 min Apgar score was higher than that of the routine group,the differences were statistically significant(P<0.05).Conclusion The seamless midwifery nursing can improve the delivery self-efficacy of primipara,shorten the labor process and improve the delivery outcome,which is worthy of further clinical promotion and application.
