摘  要:骨质疏松是骨折的常见并发症之一,因无明显临床症状,导致临床医生常忽略对其进行预防和治疗,这不仅会延长骨折愈合时间,而且极大增加了患者再次骨折的风险。研究发现,骨折后骨质疏松的发生不仅与骨折时局部组织的损伤有关,更重要的是骨折后肢体的固定导致骨折部位乃至全身的机械负荷改变使骨代谢发生变化,进而发生骨质疏松。中医认为骨折多由外伤导致机体气血损伤、气伤于内而肢损于外,脉络损伤,血溢脉外,导致气滞血瘀,进而肝、脾、肾三脏的功能受损,发展为“骨痿”“骨痹”“骨枯”,即骨折后骨质疏松。基于此,本文通过总结当前关于骨折后肢体局部和全身的生理、病理变化对骨代谢影响的文献研究,探析其发病机制、机理,为临床预防和治疗骨折后骨质疏松提供理论基础。Osteoporosis is one of the common complications of fractures.Due to the lack of obvious clinical symptoms,clinical doctors neglect its prevention and treatment,which not only prolongs the healing time of fractures,but also greatly increases the risk of patients suffering from fractures again.Research has found that the occurrence of osteoporosis after a fracture is not only related to local tissue damage during the fracture,but more importantly,the fixation of the limbs after the fracture leads to changes in mechanical load at the fracture site and even throughout the body,causing changes in bone metabolism and ultimately leading to osteoporosis.According to traditional Chinese medicine,fractures are often caused by trauma that leads to damage to the body’s qi and blood,with qi being damaged internally and limbs being damaged externally,and damage to the meridians and blood overflowing externally,resulting in qi stagnation and blood stasis,which in turn damages the functions of the liver,spleen,and kidneys and develops into"bone weakness""bone obstruction""bone necrosis",or osteoporosis after fractures.Based on this,this article summarizes the current literature research on the impact of local and systemic physiological and pathological changes in the limbs after fractures on bone metabolism,explores its pathogenesis and mechanism,and provides a theoretical basis for the clinical prevention and treatment of osteoporosis after fractures.
