摘 要:目的 探讨靶向二代测序(tNGS)在儿童支气管肺炎病原体检测中的应用价值。方法 选择2023年1-12月玉林市第一人民医院小儿内科收治的95例支气管肺炎患儿进行回顾性分析,所有患儿均行纤维支气管镜检查并采集肺泡灌洗液行病原体tNGS、微生物培养,同时所有患儿均采用化学发光法检测血清肺炎支原体抗体(MP-IgM)。分析检测结果并比较tNGS与微生物培养法、MP-IgM化学发光法对患儿病原体检出率的差异。结果 95例患儿的检测结果显示,肺泡灌洗液微生物培养法检出病原体的有16例(16.8%)。化学发光法检测MP-Ig M阳性的有21例(22.1%)。肺泡灌洗液t NGS检出病原体的有92例(96.8%),检出2种以上(含2种)病原体的有70例(73.7%)。t NGS细菌检出率(54.7%)高于微生物培养法(15.8%),差异有统计学意义(P <0.05);tNGS肺炎支原体检出率(42.1%)高于化学发光法检测MP-IgM的阳性率(22.1%),但两种检测方法的差异无统计学意义(P> 0.05)。结论 本研究中儿童支气管肺炎2种以上(含2种)混合病原体的检出率较高,tNGS在儿童支气管肺炎病原体检出方面有较高的检出率,而且对混合型感染的病原体检出具有优势,在儿童支气管肺炎的病原体检测中有较高的应用价值。Objective To investigate the application value of targeted next-generation sequencing(tNGS)in pathogen detection of pediatric bronchopneumonia.Methods A total of 95 pediatric patients with bronchopneumonia admitted to and treated in the Department of Pediatrics of the First People’s Hospital of Yulin from January to December 2023 were selected and analyzed retrospectively.All pediatric patients underwent fiberoptic bronchoscopy and collected bronchoalveolar lavage fluid for pathogen tNGS and microbial culture.Simultaneously,serum Mycoplasma pneumoniae antibodies(MP-IgM)were detected by chemiluminescence assay in all pediatric patients.The detection results were analyzed and the differences in pathogen detection rates were compared between tNGS,microbial culture method and MP-IgM chemiluminescence method in pediatric patients.Results The test results of 95 pediatric patients showed that 16 cases(accounting for 16.8%)were detected for pathogen using the bronchoalveolar lavage fluid microbial culture method.21 cases(accounting for 22.1%)were tested as positive for MP-IgM using chemiluminescence method.92 cases(accounting for 96.8%)of pulmonary lavage fluid tNGS were detected as pathogenic diseases,and 70 cases(accounting for 73.7%)were detected as two or more pathogen.The bacterial detection rate of tNGS was 54.7%,higher than 15.8%of microbial culture method,with statistically significant difference(P<0.05).The Mycoplasma pneumoniae detection rate of tNGS was 42.1%,higher than 22.1%of MP-IgM detected by chemiluminescence method,but the difference between the two detection methods was not statistically significant(P>0.05).Conclusion In this research,the detection rate of two or more mixed pathogen in children with bronchopneumonia is relatively high.tNGS has a relatively high detection rate in the detection of pathogen in children with bronchopneumonia,and has advantages in the detection of pathogen in mixed infections.It has relatively high application value in the detection of pathogen in children with bronchopne
