摘 要:目的 分析胫骨微创骨膜牵张术治疗足部慢性缺血性疾病的效果。方法 筛选阳春市人民医院2023年6月至2024年6月收治的60例足部慢性缺血性疾病患者,以入组时间分组,各30例。对照组行胫骨横向骨搬移技术治疗,观察组行胫骨微创骨膜牵张术治疗,比较趾端血氧饱和度和足背血流速度、足部周围神经和踝肱指数、临床指标。结果 观察组趾端血氧饱和度和足背血流速度优于对照组,差异有统计学意义(P <0.05);观察组足部周围神经和踝肱指数优于对照组,差异有统计学意义(P <0.05);观察组临床指标优于对照组,差异有统计学意义(P <0.05)。结论 胫骨微创骨膜牵张术的应用,可以使足部慢性缺血性疾病患者的足部疾病症状得到有效的改善,缩短住院时间,减少术中出血量,改善趾端血氧饱和度,优化足部血流速度,治疗效果显著。Objective To analyze the effect of minimally invasive periosteal distraction of tibia in the treatment of chronic ischemic diseases of foot.Methods A total of 60 patients with chronic ischemic diseases of foot admitted to and treated in Yangchun People’s Hospital from June 2023 to June 2024 were screened and divided into the control group(n=30)and the observation group(n=30)according to the time of enrollment.The control group was treated with lateral tibial bone transfer technique,while the observation group was treated with minimally invasive periosteal distraction of tibia.The oxygen saturation at toe,blood flow velocity at dorsum of foot,peripheral nerve of foot,ankle-brachial index and clinical indices were compared.Results The oxygen saturation at toe and blood flow velocity at dorsum in the observation group were superior to those in the control group,with statistically significant differences(P<0.05).The peripheral nerve of foot and ankle-brachial index in the observation group were all superior to those in the control group,with statistically significant differences(P<0.05).The clinical indices of the observation group were superior to those of the control group,with statistically significant difference(P<0.05).Conclusion The application of minimally invasive periosteal distraction of tibia can effectively improve the symptoms of foot diseases in patients with chronic ischemic diseases of foot,shorten hospitalization time,reduce intraoperative hemorrhage volume,improve oxygen saturation at toe,optimize blood flow velocity at dorsum,and achieve remarkable therapeutic efficacy.
