不同剂量丙泊酚联合右美托咪定和舒芬太尼对宫腔镜手术患者的麻醉效果

摘　　要：目的观察不同剂量丙泊酚联合右美托咪定和舒芬太尼对宫腔镜手术患者的麻醉效果。方法选取2022年6月—2023年5月金乡县人民医院收治的80例宫腔镜手术患者,按照随机数字表法分为观察组和对照组,各40例。对照组给予高剂量丙泊酚乳状注射液+盐酸右美托咪定注射液+枸橼酸舒芬太尼注射液进行麻醉,观察组给予低剂量丙泊酚乳状注射液+盐酸右美托咪定注射液+枸橼酸舒芬太尼注射液进行麻醉。比较两组血流动力学指标[心率和平均动脉压(MAP)]、复苏时间(苏醒时间和自主呼吸恢复时间)、不良反应发生情况及手术前后睡眠质量和疼痛程度[匹兹堡睡眠质量指数(PSQI)和视觉模拟评分法(VAS)]。结果观察组气管插管后、切皮后和手术结束时心率和MAP低于对照组;麻醉苏醒时间和自主呼吸恢复时间短于对照组;观察组不良反应总发生率低于对照组;术后,观察组PSQI和VAS评分低于对照组,差异有统计学意义(P<0.05)。结论低剂量丙泊酚联合右美托咪定和舒芬太尼对宫腔镜手术患者血流动力学指标和麻醉复苏时间等具有积极影响,可加快患者复苏时间,对患者术后睡眠质量和疼痛程度影响较小,且安全性较高。Objective To observe the anesthetic effect of different doses of Propofol combined with Dexmedetomidine and Sufentanil on patients undergoing hysteroscopic surgery.Methods A total of 80 patients undergoing hysteroscopic surgery admitted to Jinxiang County People's Hospital from June 2022 to May 2023 were selected and divided into observation group and control group according to the random number table method,with 40 cases in each group.The control group was given high-dose Propofol Emulsion Injection+Dexmedetomidine Hydrochloride Injection for Anesthesia+Sufentanil Citrate Injection,and the observation group was given low-dose Propofol Emulsion Injection+Dexmedetomidine Hydrochloride Injection for Anesthesia+Sufentanil Citrate Injection.The hemodynamic indexes[heart rate and mean arterial pressure(MAP)],recovery time(awakening time and spontaneous breathing recovery time),adverse reactions,sleep quality and pain degree[Pittsburgh Sleep Quality Index(PSQI)and Visual Analogue Scale(VAS)]before and after operation were compared between the two groups.Results The heart rate and MAP in the observation group were lower than those in the control group after tracheal intubation,after skin incision and at the end of operation.The anesthesia awakening time and spontaneous breathing recovery time were shorter than those in the control group.The total incidence of adverse reactions in the observation group was lower than that in the control group.After operation,the PSQI and VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusions Low-dose Propofol combined with Dexmedetomidine and Sufentanil has a positive effect on hemodynamic parameters and anesthesia recovery time in patients undergoing hysteroscopic surgery.It can accelerate the recovery time of patients,and has little effect on postoperative sleep quality and pain degree,and has high safety.
