基于冰山理论指导的个性化护理在重症肺炎患者中的应用效果

摘　　要：目的分析基于冰山理论指导的个性化护理在重症肺炎患者中的应用效果。方法选取2021年12月—2023年12月甘肃省武威肿瘤医院收治的85例重症肺炎患者,根据不同护理方法分为观察组(n=43)和对照组(n=42)。对照组采用常规护理,观察组在对照组基础上采用基于冰山理论的个性化护理。比较两组症状改善时间、负性情绪及对护理满意度。结果护理后,观察组退热、咳嗽缓解、呼吸机使用和住院时间均短于对照组,对护理总满意度高于对照组,差异有统计学意义(P<0.05)。护理前,两组汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评分比较,差异无统计学意义(P>0.05);护理后,两组HAMD和HAMA评分均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论基于冰山理论指导的个性化护理能够有效缩短重症肺炎患者症状改善时间,减轻抑郁和焦虑情绪,提高患者对护理满意度。Objective To analyze the application effect of personalized nursing based on iceberg theory in patients with severe pneumonia.Methods A total of 85 patients with severe pneumonia admitted to Gansu Wuwei Tumour Hospital from December 2021 to December 2023 were selected and divided into observation group(n=43)and control group(n=42)according to different nursing methods.The control group was given routine nursing,and the observation group was given personalized nursing based on iceberg theory on the basis of the control group.The symptom improvement time,negative emotion and nursing satisfaction were compared between the two groups.Results After nursing,the fever,cough relief,ventilator use and hospitalization time in the observation group were shorter than those in the control group,and the total satisfaction with nursing was higher than that in the control group,the differences were statistically significant(P<0.05).Before nursing,there was no significant difference in the scores of Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)between the two groups(P>0.05).After nursing,the scores of HAMD and HAMA in the two groups were lower than those before nursing,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).Conclusions Personalized nursing based on iceberg theory can effectively shorten the time of symptom improvement in patients with severe pneumonia,reduce depression and anxiety,and improve patients'satisfaction with nursing.
