督脉灸联合针刀治疗强直性脊柱炎的临床观察

摘　　要：目的旨在以“贺氏三通法”理论为指导,探讨督脉灸联合针刀治疗强直性脊柱炎的临床疗效。方法选择30例就诊于石嘴山市中医院强直性脊柱炎患者,进行“温通法”督脉灸及“强通法”针刀治疗。隔3天治疗1次,3次为1疗程,治疗1个疗程。分别于治疗前后观察患者中医证候、Bath强直性脊柱炎疾病活动性指数(BASDAI)、Bath强直性脊柱炎机体功能性活动量化指数(BASFI)、骨骼和肌肉系统活动度及脊柱疼痛VAS评分。结果通过治疗,患者的中医症状评分较前降低,差异有统计学意义(P<0.05);患者的BASFI评分、BASDAI评分、枕墙距、胸廓活动度、指地距评分均低于治疗前,Schober试验评分高于治疗前,差异有统计学意义(P<0.05);患者脊柱疼痛VAS评分低于治疗前,差异具有统计学意义(P<0.05)。结论以“贺氏三通法”为理论指导,督脉灸联合针刀治疗能有效缓解伴脊柱活动受限AS患者项背拘挛、腰背部僵硬、疼痛及活动受限,从而减轻脊柱疼痛、晨僵及功能活动障碍,改善脊柱生理功能和提高生活质量。Objective:To explore the clinical efficacy of Du Mai acupuncture combined with acupotomy therapy for ankylosing spondylitis(AS),guided by the“He's San Tong Method”theory.Methods:30 AS patients who visited Shizuishan Hospital of Traditional Chinese Medicine were selected and treated with Du Mai acupuncture based on the“Wen Tong Fa”method and acupotomy therapy based on the“Qiang Tong Fa”method.The treatment was given every three days for one course,and one course consisted of 3 treatments.The TCM symptoms,Bath Ankylosing Spondylitis Disease Activity Index(BASDAI),Bath Ankylosing Spondylitis Functional Index(BASFI),skeletal and muscular activity and spinal pain Visual Analog Scale(VAS)scores were observed before and after treatment.Results:After treatment,the TCM symptoms score decreased significantly,and the difference was statistically significant(P<0.05).The BASFI,BASDAI,occiput-wall distance,thoracic motion and finger-to-floor distance scores were all significantly lower than those before treatment while the Schober test score was higher than that before treatment,with the difference being statistically significant(P<0.05).The spinal pain VAS score was also significantly reduced with the difference being statistically significant(P<0.05).Conclusion:Guided by the“He's San Tong Method,”Du Mai acupuncture combined with acupotomy therapy effectively alleviates the symptoms of AS patients,including neck and back stiffness,pain and limited movement,thereby reducing pain,morning stiffness and functional impairment and improving spinal physiological function and quality of life.
