丹栀逍遥散联合重复经颅磁刺激治疗原发性失眠的疗效研究及机制探讨

摘　　要：目的观察原发性失眠以丹栀逍遥散与重复经颅磁刺激(rTMS)联合治疗的临床效果,并对其可能作用机制进行探讨分析。方法将辨证为肝郁化火证的原发性失眠患者60例,采用完全随机对照原则分为2组,每组30例。对照组给予重复经颅磁刺激治疗,治疗组在对照组基础上加上丹栀逍遥散。结果治疗后,2组患者PSQI评分、ISI评分与治疗前相比均有明显下降(P<0.05),且治疗组的评分下降幅度优于对照组(P<0.05);2组患者血清GABA、BDNF浓度较治疗前有所上升(P<0.05);2组中医症状积分均有下降(P<0.05),且治疗组下降幅度优于对照组(P<0.05);治疗组的疗效优于对照组,差异有统计学意义(P<0.05)。结论丹栀逍遥散联合重复经颅磁刺激(rTMS)治疗原发性失眠疗效确切,同时能调节血清神经递质浓度。Objective:To observe the clinical efficacy of Danzhi Xiaoyao Pewder combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of primary insomnia and explore its possible mechanisms.Methods:Sixty patients with primary insomnia due to liver depression and fire syndrome were randomly divided into two groups,30 cases per group.The control group received rTMS treatment while the treatment group received rTMS combined with Danzhi Xiaoyao Powder.Results:After treatment,the Pittsburgh Sleep Quality Index(PSQI)and Insomnia Severity Index(ISI)scores significantly decreased in both groups(P<0.05),with greater reductions in the treatment group(P<0.05).Serum levels of gamma-aminobutyric acid(GABA)and brain-derived neurotrophic factor(BDNF)increased in both groups(P<0.05).TCM symptom scores decreased significantly,with a greater reduction in the treatment group(P<0.05).The treatment group showed superior overall efficacy compared to the control group and the difference had statistical significance(P<0.05).Conclusion:Danzhi Xiaoyao Powder combined with rTMS is effective for treating primary insomnia and meanwhile can modulate serum neurotransmitter concentrations.
