摘 要:目的明确排毒保肾丸治疗慢性肾脏病(chronic kidney disease,CKD)的作用机制。方法SPF级雄性SD大鼠随机分为假手术组、模型组、排毒保肾丸组及尿毒清颗粒组4组。采用5/6肾切除模型建立CKD。成模后,排毒保肾丸组给予排毒保肾丸(0.54g/kg),尿毒清组给予尿毒清(1.35g/kg)治疗,假手术组和模型组予等容积蒸馏水,每天1次,连续8周。给药结束后,取肾及结肠,采用HE、PAS和Masson染色行病理学检查;取粪便,采用16S rDNA测序并进行生信分析。结果排毒保肾丸治疗可显著缓解5/6肾切除模型大鼠的肾脏和结肠病理损伤(P<0.05),并可调节肠道菌群,显著提高益生菌Prevotellaceae(普氏菌科)、Akkermansia(黏液杆菌)、Phascolarctobacterium(芽孢杆菌属)的相对丰度,同时显著降低Clostridiaceae(梭菌科)、Hungateiclostridiaceae(红毛菊科)、Ruminococcus(瘤胃球菌属)的相对丰度(P<0.05),表明排毒保肾丸通过调节肠道菌群,抑制炎症,修复肠道及肾脏组织病理,延缓5/6肾切除大鼠肾功能进展。结论排毒保肾丸通过调节肠道菌群,抑制炎症,修复肠道及肾脏组织病理,具有延缓CKD进展的作用。Objective:To elucidate the mechanism of Paidu Baoshen Pills in the treatment of chronic kidney disease(CKD).Methods:Male SPF-grade SD rats were randomly divided into four groups:sham surgery group,model group,Paidu Baoshen Pills group,and Niaoduqing Granules group.A 5/6 nephrectomy model was used to establish CKD.After model establishment,the Paidu Baoshen Pills group received Paidu Baoshen Pills(0.54 g/kg),the Niaoduqing group received Niaoduqing Granules(1.35 g/kg)and the sham surgery and model groups received an equal volume of distilled water,administered once daily for eight weeks.After treatment,kidney and colon tissues were collected for histopathological analysis by using HE,PAS and Masson staining.Fecal samples were subjected to 16S rDNA sequencing and bioinformatics analysis.Results:Paidu Baoshen Pills significantly alleviated renal and colonic pathological damage in 5/6 nephrectomized rats(P<0.05)and modulated gut microbiota composition.The treatment significantly increased the relative abundance of beneficial bacteria,including Prevotellaceae,Akkermansia,and Phascolarctobacterium,while significantly decreasing the relative abundance of Clostridiaceae,Hungateiclostridiaceae,and Ruminococcus(P<0.05).These findings suggest that Paidu Baoshen Pills exert their therapeutic effects by modulating gut microbiota,reducing inflammation and repairing intestinal and renal tissue damage,thereby slowing CKD progression.Conclusion:Paidu Baoshen Pills effectively regulate gut microbiota,inhibit inflammation and repair tissue damage in the intestines and kidneys,demonstrating potential in delaying CKD progression.
