开郁安神养肾汤对心肾不交型抑郁症患者的疗效及神经递质水平的影响

摘　　要：目的分析开郁安神养肾汤对心肾不交型抑郁症患者的疗效及神经递质水平的影响。方法方便选取2021年1月—2023年12月定西市人民医院收治的76例心肾不交型抑郁症患者作为研究对象。以不同用药方案分为观察组与对照组,各38例。对照组使用草酸艾司西酞普兰片治疗,观察组在对照组基础上使用开郁安神养肾汤。对比两组临床疗效、抑郁症程度、神经递质水平。结果观察组临床治疗总有效率为92.11%(35/38),高于对照组71.05%(27/38),差异有统计学意义(χ^(2)=5.604,P=0.018)。治疗前,两组症状自评量表(Symptom Checklist-90,SCL-90)、汉密尔顿抑郁量表(Hamilton Depression Rating Scale,HAMD-17)评分比较,差异均无统计学意义(P均>0.05);治疗后,两组SCL-90、HAMD-17评分均较治疗前降低,且观察组低于对照组,差异均有统计学意义(P均<0.05)。治疗前,两组5-羟色胺(5-hydroxytryptamine,5-HT)、去甲肾上腺素(norepinephrine,NE)水平比较,差异均无统计学意义(P均>0.05)。治疗后,两组5-HT、NE水平均较治疗前升高,且观察组高于对照组,差异均有统计学意义(P均<0.05)。结论对心肾不交型抑郁症患者,在应用草酸艾司西酞普兰片治疗的同时,联合使用开郁安神养肾汤能显著提高其临床疗效,改善抑郁程度。Objective To analyze the effect of Kaiyu Anshen Yangshen Decoction on the efficacy and neurotransmitter levels in patients with depression of disharmony between heart and kidney type.Methods A total of 76 patients with depression of disharmony between heart and kidney type who were admitted to Dingxi People's Hospital from January 2021 to December 2023 were conveniently selected as the research objects.They were divided into the observation group and the control group according to different medication regimens,with 38 cases in each group.The control group was treated with escitalopram oxalate tablets,while the observation group was treated with Kaiyu Anshen Yangshen Decoction on the basis of the control group.The clinical efficacy,degree of depression and neurotransmitters were compared between the two groups.Results The total clinical effective rate of the observation group was 92.11%(35/38),which was higher than that of the control group[71.05%(27/38)],and the difference was statistically significant(χ^(2)=5.604,P=0.018).Before treatment,there were no statistically significant differences in the scores of the Symptom Checklist-90(SCL-90)and the Hamilton Depression Rating Scale(HAMD-17)between the two groups(both P>0.05);after treatment,the scores of SCL-90 and HAMD-17 in both groups were lower than those before treat ment,and those in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).Before treatment,there were no statistically significant differences in the levels of 5-hydroxytryptamine(5-HT)and norepinephrine(NE)between the two groups(both P>0.05).After treatment,the levels of 5-HT and NE in both groups were higher than those before treatment,and those in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion For patients with depression of disharmony between heart and kidney type,when treated with escitalopram oxalate tablets,combined with the Kai
