单、双切口双钢板固定术在胫腓骨远端骨折患者中的应用效果

摘　　要：目的研究单、双切口双钢板固定术在胫腓骨远端骨折患者中的应用效果。方法随机选取2021年1月—2024年1月福清市第三医院收治的300例胫腓骨远端骨折的患者作为研究对象,依据不同手术方式分为两组,各150例。对照组行双切口双钢板固定术处理,观察组行单切口双钢板固定术处理,比较两组的围手术期基础指标、患肢功能、手术优良率及其安全性。结果观察组手术时间、骨痂形成时间及住院时间均短于对照组,术中失血量、手术并发症总发生率均比对照组更低,差异均有统计学意义(P均<0.05)。术后末次随访,观察组Johner-Wruhs得分高于对照组,差异有统计学意义(P<0.05)。相较于对照组,观察组手术优良率更高[94.00%(141/150)比82.67%(124/150)],差异有统计学意义(χ^(2)=9.348,P=0.002)。结论单切口双钢板固定术用于胫腓骨远端骨折患者的处理近期与远期效果相对较好,相比双切口双钢板固定术治疗更利于促进患肢功能恢复,并提高手术安全性。Objective To study the application effect of single and double incision double plate fixation in patients with distal tibiofibular fractures.Methods From January 2021 to January 2024,300 patients with distal tibiofibular fractures admitted to Fuqing Third Hospital were randomly selected as the research objects.According to different surgical methods,they were divided into two groups with 150 cases in each group.Double incision and double plate fixation was used in the control group and single incision and double plate fixation was used in the observation group.The perioperative basic indexes,limb function,excellent and good rate of operation and safety were compared between the two groups.Results The observation group's operation time,scab formation time and hospital stay were shorter than the control group,and the intraoperative blood loss and the total incidence of surgical complications were lower than those of the control group,with statistically significant differences(all P<0.05).At the last postoperative follow-up,the observation group had a higher Johner-Wruhs score than the control group,and the difference was statistically significant(P<0.05)In terms of surgical excellence,the observation group was higher than the control group[94.00%(141/150)vs 82.67%(124/150)],the difference was statistically significant(χ^(2)=9.348,P=0.002).Conclusion The short-term and long-term effects of single incision double plate fixation for the treatment of patients with distal tibiofibular fractures are better,compared with double incision double plate fixation,it is more conducive to promoting the recovery of limb function and improving the safety of surgery.
