摘 要:目的探讨糖尿病肾病(diabetic kidney disease,DKD)患者血清甲状腺激素与临床分期的相关性。方法选取2022年9月—2024年6月在柳州市红十字会医院接受治疗的240例DKD患者,根据肾小球滤过率、尿微量白蛋白/尿肌酐比值、慢性肾脏病进展风险的高低分为低风险组(55例)、中风险组(63例)、高风险组(67例)、极高风险组(55例)。比较4组患者一般资料、血清甲状腺激素水平,分析血清甲状腺激素与肾脏临床分期的相关性。结果中、高、极高风险组血清促甲状腺激素(thyroid stimulating hormone,TSH)、体重指数、大量白蛋白尿占比均高于低风险组,血清游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)均低于低风险组,差异均有统计学意义(P均<0.05)。相关性分析显示,血清TSH与DKD临床分期呈正相关,FT3、FT4与DKD临床分期呈负相关。结论血清FT3、FT4、TSH水平与DKD临床分期密切相关。Objective To investigate the correlation between serum thyroid hormone and clinical stage in patients with diabetic kidney disease(DKD).Methods A total of 240 patients with DKD who were treated in Liuzhou Red Cross Hospital from September 2022 to June 2024 were selected.According to glomerular filtration rate,urine microalbumin/creatinine ratio and the risk of chronic kidney disease progression,they were divided into low-risk group(55 cases),medium-risk group(63 cases),high-risk group(67 cases)and very high-risk group(55 cases).The general data and serum thyroid hormone levels of the four groups were compared,and the correlation between serum thyroid hormone and renal clinical stage was analyzed.Results The levels of serum thyroid stimulating hormone(TSH),body mass index and the proportion of macroalbuminuria in the middle-risk,high-risk and very high-risk groups were higher than those in the low-risk group,and the levels of serum free triiodothyronine(FT3)and free thyroxine(FT4)were lower than those in the low-risk group,the differences were statistically significant(all P<0.05).Correlation analysis showed that serum TSH was positively correlated with clinical stage of DKD,FT3 and FT4 were negatively correlated with clinical stage of DKD.Conclusion Serum FT3,FT4 and TSH levels are closely related to the clinical stage of DKD.
