德谷胰岛素利拉鲁肽注射液治疗口服药物控制不佳2型糖尿病的作用分析

摘　　要：目的探究德谷胰岛素利拉鲁肽注射液用于口服药控制不佳2型糖尿病(type 2 diabetes mellitus,T2DM)患者中的效果及其应用价值。方法选取2023年3月—2024年8月厦门医学院附属海沧医院收治的100例T2DM患者,按治疗方法的不同分为对照组、观察组,各50例。对照组采用二甲双胍联合德谷门冬双胰岛素治疗,观察组采用二甲双胍联合德谷胰岛素利拉鲁肽注射液治疗。比较两组血糖水平、生化指标、胰岛功能、临床疗效及不良反应发生情况。结果治疗后,与对照组相比,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、甘油三酯、总胆固醇水平、体重指数、胰岛素抵抗指数均更低,胰岛β细胞功能指数更高,差异均有统计学意义(P均<0.05)。与对照组相比,观察组治疗总有效率更高,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论德谷胰岛素利拉鲁肽注射液可促进胰岛素分泌、提高胰岛素敏感性等,进而降低体重及改善综合代谢,有效提高了T2DM患者的临床疗效。Objective To explore the effect and application value of insulin degludec liraglutide injection in patients with type 2 diabetes mellitus(T2DM)with poor oral drug control.Methods A total of 100 patients with T2DM admitted to Haicang Hospital Affiliated to Xiamen Medical College from March 2023 to August 2024 were selected and divided into control group and observation group according to different treatment methods,with 50 cases in each group.The control group was treated with metformin combined with degludec insulin aspart,and the observation group was treated with metformin combined with insulin degludec liraglutide injection.The blood glucose level,biochemical indexes,islet function,clinical efficacy and adverse reactions were compared between the two groups.Results After treatment,compared with the control group,the levels of fasting plasma glucose,2-hour postprandial blood glucose,glycated hemoglobin A1c,triglyceride,total cholesterol,body mass index and homeostasis model assessment for insulin resistance in the observation group were lower,and the homeostasis model assessment-βcell function index was higher,the differences were statistically significant(all P<0.05).Compared with the control group,the total effective rate of the observation group was higher,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Insulin degludec liraglutide injection can enhance insulin secretion and improve insulin sensitivity,thereby reducing body weight and improving comprehensive metabolism,effectively improving the clinical efficacy of T2DM patients.
