达格列净联合二甲双胍与甘精胰岛素对老年2型糖尿病患者糖脂代谢、胰岛素抵抗和体重指数的影响

摘　　要：目的评价甘精胰岛素治疗老年2型糖尿病患者对比达格列净+二甲双胍治疗对糖脂代谢、胰岛素抵抗和氧化应激的影响。方法选取2023年1月—2024年5月于霞浦县医院就诊的老年2型糖尿病患者90例,按治疗方法不同分为对照组(甘精胰岛素)和观察组(达格列净联合二甲双胍),各45例,对比两组患者血糖水平、血脂水平、胰岛素抵抗指数和体重指数。结果治疗后,与对照组相比,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、总胆固醇、甘油三酯水平、胰岛素抵抗指数、体重指数均更低,差异均有统计学意义(P均<0.05)。结论达格列净+二甲双胍方案联合治疗老年2型糖尿病整体疗效更优,能改善患者的血糖水平,减轻胰岛素抵抗。Objective To evaluate the effects of insulin glargine on glucose and lipid metabolism,insulin resistance and oxidative stress in elderly patients with type 2 diabetes mellitus compared with dapagliflozin+metformin.Methods A total of 90 elderly patients with type 2 diabetes mellitus in Xiapu County Hospital from January 2023 to May 2024 were selected and divided into control group(insulin glargine)and observation group(dapagliflozin combined with metformin)according to different treatment methods,with 45 cases in each group.The blood glucose level,blood lipid level,insulin resistance index and body mass index were compared between the two groups.Results After treatment,compared with the control group,the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,total cholesterol,triglyceride,insulin resistance index and body mass index of the observation group were lower,and the differences were statistically significant(all P<0.05).Conclusion The combination of dapagliflozin+metformin is more effective in the treatment of elderly patients with type 2 diabetes mellitus,which can improve the blood glucose level and reduce the insulin resistance.
