摘 要:目的研究门冬胰岛素联合盐酸二甲双胍片对妊娠期糖尿病孕妇糖代谢水平、炎症因子及治疗效果的影响。方法选取2023年2—8月灌云县妇幼保健计划生育服务中心登记在册并纳入风险管理的57例妊娠期糖尿病孕妇,根据不同治疗方法分为MD组(29例,以门冬胰岛素治疗)和MD+MHT组(28例,采用门冬胰岛素联合盐酸二甲双胍片治疗)。比较两组糖代谢水平、炎性因子、治疗总有效率。结果治疗后,两组孕妇空腹血糖、餐后2 h血糖、糖化血红蛋白、超敏C反应蛋白、白细胞介素-6、肿瘤坏死因子均较治疗前降低,MD+MHT组较MD组降低更显著,差异均有统计学意义(P均<0.05)。MD+MHT组治疗总有效率为92.86%(26/28),高于MD组的72.41%(21/29),差异有统计学意义(χ^(2)=4.626,P=0.031)。结论门冬胰岛素联合盐酸二甲双胍片可提高妊娠期糖尿病孕妇糖代谢水平,降低机体炎症反应,有效控制不良妊娠结局发生。Objective To study the effect of insulin aspart combined with metformin hydrochloride tablets on glucose metabolism,inflammatory factors and therapeutic effect in pregnant women with gestational diabetes mellitus.Methods From February to August 2023,57 pregnant women with gestational diabetes mellitus who were registered and included in risk management in Maternal and Child Health and Family Planning Service Center of Guanyun County were selected.According to different treatment methods,they were divided into MD group(29 cases,treated with insulin aspart)and MD+MHT group(28 cases,treated with insulin aspart combined with metformin hydrochloride tablets).The levels of glucose metabolism,inflammatory factors and total effective rate of treatment were compared between the two groups.Results After treatment,the fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,hypersensitive C-reactive protein,interleukin-6 and tumor necrosis factor in the two groups were lower than those before treatment,and those of the MD+MHT group were lower than the MD group,the differences were statistically significant(all P<0.05).The total effective rate of treatment in the MD+MHT group was 92.86%(26/28),which was higher than 72.41%(21/29)in the MD group,and the difference was statistically significant(χ^(2)=4.626,P=0.031).Conclusion Insulin aspart combined with metformin hydrochloride tablets can improve the level of glucose metabolism in pregnant women with gestational diabetes mellitus,reduce the body's inflammatory response,and effectively control the occurrence of adverse pregnancy outcomes.
