二甲双胍与达格列净片联合治疗对老年糖尿病的临床价值

摘　　要：目的评估二甲双胍联合达格列净片应用在老年糖尿病(diabetes mellitus,DM)患者治疗中的临床价值。方法选取2023年6月—2024年9月上海市嘉定区迎园医院收治的106例老年DM患者,根据不同的治疗方法分为两组,对照组(53例)进行二甲双胍治疗,观察组(53例)进行二甲双胍联合达格列净片治疗。比较两组糖代谢指标、胰岛功能指标、微炎症状态、不良反应发生率。结果治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、空腹胰岛素、胰岛β细胞功能指数、胰岛素抵抗指数、肿瘤坏死因子-α、白细胞介素-6、C反应蛋白水平均优于对照组,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论老年DM患者采用二甲双胍联合达格列净片治疗可以改善糖代谢指标、胰岛功能,减轻微炎症状态,安全性良好。Objective To evaluate the clinical value of metformin combined with dapagliflozin tablets in the treatment of elderly patients with diabetes mellitus(DM).Methods A total of 106 elderly patients with DM admitted to Shanghai Jiading District Yingyuan Hospital from June 2023 to September 2024 were selected and divided into two groups according to different treatment methods.The control group(53 cases)was treated with metformin,and the observation group(53 cases)was treated with metformin combined with dapagliflozin tablets.The glucose metabolism indexes,islet function indexes,micro-inflammatory state and incidence of adverse reactions were compared between the two groups.Results After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,fasting insulin,homeostasis model assessment-β,homeostasis model assessment-insulin resistance,tumor necrosis factor-α,interleukin-6 and C-reactive protein in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Metformin combined with dapagliflozin tablets in elderly patients with DM can improve glucose metabolism indicators,islet function,reduce microinflammatory state,and have good safety.
