广西地区艰难梭菌谷氨酸脱氢酶抗原及毒素检测试剂盒的检出率及性能评价

摘　　要：目的对广西地区艰难梭菌谷氨酸脱氢酶抗原及毒素检测试剂盒的检出率及性能进行评价。方法收集广西壮族自治区江滨医院2023年1月至2024年4月因腹泻收治的患者粪便样本416份,以毒素培养法的结果为金标准,使用艰难梭菌谷氨酸脱氢酶抗原及毒素检测试剂盒和艰难梭菌毒素A/B检测试剂盒。比较两种试剂盒的阴性预测值、阳性预测值及特异度、敏感度。结果广西地区艰难梭菌的阳性率为19.7%(82/416)。艰难梭菌谷氨酸脱氢酶抗原及毒素检测试剂盒的阴性92.55%、阳性预测值86.78%及特异度98.80%、敏感度56.73%均高于艰难梭菌毒素A/B检测试剂盒的90.63%、66.58%、95.91%、51.44%(P<0.05)。结论广西地区腹泻患者中病毒检出率较高。艰难梭菌谷氨酸脱氢酶抗原及毒素检测试剂盒特异度和敏感度均较高,阳性预测值和阴性预测值高。Objective To evaluate the detection rate and performance of Clostridioides difficile glutamate dehydrogenase antigen and toxin detection kits in Guangxi region.Methods A total of 416 fecal samples were collected from patients admitted to Jiangbin Hospital of Guangxi Zhuang Autonomous Region due to diarrhea from January 2023 to April 2024.For all the diarrhea fecal specimens of the subjects included in the study,the gold standard of this study was the result of toxin culture method.Two kits were used:Clostridioides difficile glutamate dehydrogenase antigen and toxin detection kit,and Clostridioides difficile toxin A/B detection kit.The negative predictive value,positive predictive value,specificity and sensitivity of the two kits were compared.Results The positive rate of Clostridioides difficile in Guangxi region was 19.7%(82/416).The negative predictive value(92.55%),positive predictive value(86.78%),specificity(98.80%)and sensitivity(56.73%)of the Clostridioides difficile glutamate dehydrogenase antigen and toxin detection kit were all higher than those of the Clostridioides difficile toxin A/B detection kit(90.63%,66.58%,95.91%,51.44%)(P<0.05).Conclusions The detection rate of the virus is relatively high among diarrhea patients in Guangxi region.The Clostridioides difficile glutamate dehydrogenase antigen and toxin detection kit has high specificity and sensitivity,as well as high positive and negative predictive values.
