黄河清教授中医治疗肺结节的理论及临床应用的再思考

摘　　要：手术治疗是直径较大或恶性风险高的肺结节的主要治疗手段,定期CT随访是直径较小的肺结节的主要干预手段。中医药干预早期肺结节具有巨大优势及空间。黄河清教授认为,肺结节为以气虚为本,综合了气滞、瘀血、痰凝等标实因素,多种外因共同作用而成的病机复杂的疾病,其核心病机为气结痰瘀、坚积于肺。根据此病机,黄河清教授自拟主方“桂枝茯苓散结方”,并提出了因病立方、辨证施治、随症加减治疗体系,本文对此进行了综述。Surgical treatment is the main therapeutic measure for lung nodules with a large diameter or high malignant risk,while regular CT follow-up is the main intervention measure for lung nodules with a small diameter.Traditional Chinese medicine(TCM)intervention has great potential and space in the early intervention of lung nodules.The TCM mechanisms of lung nodules as recognized by Professor Huang Heqing,who believes that lung nodules are a complex disease with a core mechanism of qi stagnation,phlegm stasis,and blood stasis,caused by a combination of external factors such as qi deficiency,blood stasis,and phlegm congestion.Based on this mechanism,the author formulated the main prescription"Guizhi Tuckahoe Sanjie Formula"and proposed a treatment system of formulating prescriptions according to the disease,individualized treatment,and adding or reducing medications according to symptoms.Professor Huang Heqing believes that pulmonary nodules are complex diseases with a fundamental deficiency of qi and a combination of various pathogenic factors such as qi stagnation,blood stasis,and phlegm coagulation.The core pathogenesis is the accumulation of qi stagnation,phlegm,and blood stasis in the lungs.Based on this pathogenesis,Professor Huang Heqing has developed a self-designed main formula,"Guizhi Tuckahoe Sanjie Decoction",and proposed a treatment system of formulating prescriptions based on the disease,differentiating syndromes for treatment,and adding or subtracting according to symptoms.This article provides a review of this.
