摘 要:目的分析在妊娠期糖尿病预测中孕早期空腹血糖、BMI联合检查的应用价值。方法选取2021年1月至2024年6月在武夷山市洋庄乡卫生院妇产科规律孕检的76例孕妇。分析孕早期空腹血糖、BMI,在妊娠24~28周时通过口服葡萄糖耐量试验(OGTT)判断不同孕妇是否存在妊娠期糖尿病,收集并整理不同孕妇基本信息、病史资料、实验室检查等数据资料。结果76例孕妇中36例为妊娠期糖尿病,40例为健康孕妇。妊娠期糖尿病患者年龄、孕早期空腹血糖、孕早期BMI差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,孕早期空腹血糖、BMI是妊娠期糖尿病发生的独立危险因素(P<0.01)。通过构建模型,并计算ROC曲线下面积,最佳空腹血糖切点值为4.59 mmol/L时,此时对妊娠期糖尿病的预测灵敏度、特异性分别为79.50%、71.38%;BMI的最佳切点值为23.15 kg/m^(2),对妊娠期糖尿病的预测灵敏度、特异性分别为68.40%、63.95%。结论孕早期空腹血糖联合BMI可作为预测妊娠期糖尿病发生风险的重要指标。在孕早期空腹血糖与BMI较高的情况下,孕妇发生妊娠期糖尿病的风险较高,可结合不同孕妇早期空腹血糖以及BMI情况做好早期干预,以降低妊娠期糖尿病发病风险。Objective To analyze the combined value of fasting blood glucose and body mass index(BMI)in the prediction of gestational diabetes mellitus.Methods A total of 76 pregnant women who underwent regular prenatal check-ups at the Department of Obstetrics and Gynecology of Yangzhuang were selected.Township Health Center in Wuyishan City from January 2021 to June 2024.Analyze early fasting blood glucose,BMI,in 24 to 28 weeks of pregnancy by oral glucose tolerance test(OGTT)determine whether different pregnant women have gestational diabetes,collect and organize different basic information,medical history,laboratory examination and other data.Results Of the 76 pregnant women,36 were gestational diabetes mellitus and 40 were healthy pregnant women.Age at gestational diabetes,pregnancy time,delivery time,early pregnancy fasting blood glucose,and early pregnancy BMI were greater than the control group and had significant differences(P<0.05).Multivariate Logistic regression analysis showed that fasting blood glucose and BMI in early pregnancy were independent risk factors for gestational diabetes mellitus(P<0.01).By constructing the model,the best fasting glucose cut-point value of 4.59 mmol/L and the optimal cut-point value of BMI is 23.15 kg/m^(2).Conclusions The fasting blood glucose combined with BMI in early pregnancy can be used as an important indicator to predict the risk of diabetes mellitus in pregnancy.In the case of high fasting blood glucose and BMI in the early pregnancy,pregnant women have a higher risk of gestational diabetes.Early intervention can be combined with different early fasting blood glucose and BMI to reduce the risk of gestational diabetes.
