摘 要:目的探讨脊神经后支脉冲射频联合药物毁损对于胸腰椎压缩性骨折所致疼痛的临床效果。方法按照随机数字表法,选取2019年7月至2023年6月于重庆市东南医院接受脊神经后支脉冲射频联合药物毁损治疗的胸腰椎压缩性骨折疼痛患者100例为观察组,选取同期予以保守治疗的胸腰椎压缩性骨折疼痛患者100例为对照组。对比数字疼痛评分法(NRS)、Oswestry功能障碍指数(ODI)、匹兹堡睡眠指数量表(PSQI)评分。结果观察组患者接受治疗后1 d、1个月、3个月的时间节点进行评估所得到的NRS评分、ODI评分、PSQI评分均低于对照组(P<0.05)。两组患者中仅出现2例便秘患者,无其他并发症,经对症处理后缓解。结论对于胸腰椎压缩性骨折患者,进行脊神经后支脉冲射频联合药物毁损,具有理想的镇痛效果,有利于改善患者睡眠质量和提高腰椎功能。Objective To explore the clinical effect of pulse radiofrequency of posterior spinal nerve branches combined with pharmacological destruction in pain caused by thoracolumbar vertebral compression fracture.Methods A total of 100 patients with thoracolumbar compression fracture pain who received pulsed radiofrequency combined with drug neurolysis treatment of the posterior spinal nerve at Southeast Hospital of Chongqing from July 2019 to June 2023 were selected as the observation group,and 100 patients with thoracolumbar compression fracture pain who received conservative treatment during the same period were selected as the control group.The study compared the two groups in terms of the Numeric Rating Scale(NRS),Oswestry Disability Index(ODI),and Pittsburgh Sleep Quality Index(PSQI)scores.Results The NRS score,ODI score,and PSQI score obtained at the time points of 1 day,1 month,and months after treatment were lower in the observation group than in the control group(P<0.05).Only 2 of constipation occurred in the two groups of patients,and there were no other complications,which were relieved after symptomatic treatment.Conclusions For patients with thoracolumbar compression fractures,performing pulse radiofrequency of the posterior branch of the spinal nerve combined with drug ablation has idealgesic effects,which is conducive to improving the sleep quality of patients and improving lumbar function.
