摘　　要：目的分析来氟米特联合甲氨蝶呤治疗类风湿性关节炎的治疗效果。方法选取泗洪县第一人民医院与泗洪分金亭医院2020年2月至2024年3月收治的105例类风湿性关节炎患者,经随机摸球法分为两组,对照组52例,研究组53例,对照组口服甲氨蝶呤片治疗,研究组在对照组基础上联合来氟米特治疗。分别于治疗前及治疗3个月对比两组炎症反应(血沉、类风湿因子、C反应蛋白)、关节功能、生活质量[生活质量简表(WHOQOL-100)]。结果治疗后,研究组治疗总有效率较对照组高(P<0.05)。治疗后,研究组类风湿因子水平、血沉水平、C反应蛋白水平较对照组低(均P<0.05)。治疗后,研究组关节功能分级中,Ⅰ级患者26例,Ⅱ级患者11例,Ⅲ级患者15例,Ⅳ级患者1例,对照组组关节功能分级中,Ⅰ级患者18例,Ⅱ级患者9例,Ⅲ级患者16例,Ⅳ级患者9例,研究组关节功能高于对照组(P<0.05)。治疗后,研究组WHOQOL-100各项评分较对照组高(均P<0.05)。结论使用来氟米特联合甲氨蝶呤治疗类风湿性关节炎,两种药物作用机制互补,能够降低炎症因子水平,改善关节功能,从而提升其生活质量。Objective Analyze clinical efficacy of leflunomide combined with methotrexate in the treatment of rheumatoid arthritis and the effect on joint function of patients.Methods A total of 105 rheumatoid arthritis patients treated at two hospitals in Sihong County from February 2020 to March 2024 were selected and divided into two groups using random ball drawing.The control group(52 patients)received oral methotrexate,while the study group(53 patients)received methotrexate plus leflunomide.Clinical efficacy and inflammatory markers(erythrocyte sedimentation rate,rheumatoid factor,C-reactive protein),joint function,and quality of life(assessed by WHOQOL-100)were compared between groups before and 3 months after treatment.Results After treatment,the study group had a higher overall effective rate than the control group(P<0.05).The levels of rheumatoid factor,erythrocyte sedimentation rate,and C-reactive protein were lower in the study group than in the control group(all P<0.05).Joint function grading showed that the study group had more patients at higher grades(26 at GradeⅠ,11 at GradeⅡ,15 at GradeⅢ,1 at GradeⅣ)compared to the control group(18 at GradeⅠ,9 at GradeⅡ,16 at GradeⅢ,9 at GradeⅣ),indicating superior joint function in the study group(P<0.05).Additionally,the WHOQOL-100 score was significantly higher in the study group(P<0.05).Conclusions Leflunomide combined with methotrexate has high clinical efficacy in the treatment of rheumatoid arthritis.The mechanism of action of the two drugs is complementary,which can reduce the level of inflammatory factors and improve the joint function of patients,so as to improve their quality of life.
