肩关节镜手术患者应用超声引导下肌间沟神经阻滞联合全身麻醉干预的效果研究

摘　　要：目的探究肩关节镜手术患者应用超声引导下肌间沟神经阻滞联合全身麻醉干预的效果。方法选取2020年1月至2024年5月至江苏大学附属武进医院就诊的肩关节镜手术患者,共87例,经随机数字表法将其分为对照组(44例,采用全身麻醉干预)、观察组(43例,采用全身麻醉后行臂丛神经阻滞),对比其血流动力学、血清学指标及不良反应发生率。结果观察组患者麻醉后15 min及术毕收缩压、舒张压、平均动脉压、心率等血流动力学指标水平均低于对照组(均P<0.05)。手术30 min及拔管10 min后血清皮质醇及去甲肾上腺素等血清学指标水平均低于对照组(均P<0.05)。观察组患者发生不良反应总发生率低于对照组(P<0.05)。结论超声引导下肌间沟神经阻滞联合全身麻醉干预在改善肩关节镜手术患者血流动力学、稳定血清学指标和不良反应发生率方面效果更加显著。Objective To explore effect ultrasound-guided intermuscular groove nerve block general anesthesia intervention undergoing shoulder arthroscopy surgery.Methods A total of 87 patients undergoing shoulder arthroscopy surgery from January 2020 to May 2024 were divided into control group(44 cases,using general anesthesia intervention)by random number chart,observation group(43 cases,using brachial plexus block after general anesthesia),compare the hemodynamics,serum indicators,and incidence of adverse reactions.Results In the observation group,the level of hemodynamic indexes such as systolic blood pressure,diastolic blood pressure,mean arterial pressure,and heart rate at 15min after anesthesia(all P<0.05).At 30 minutes after surgery and 10 minutes after extubation,the levels of serum cortisol and norepinephrine,among other serum indicators,were significantly lower than those in the control group(all P<0.05).The incidence of adverse reactions in the observation group was lower than that of the control group(P<0.05).Conclusions Ultrasound-guided interscalene nerve block combined with general anesthesia intervention was more effective in improving hemodynamics,stable serological indexes and incidence of adverse effects in patients undergoing shoulder arthroscopic surgery.
