早期糖尿病肾病诊断中尿微量白蛋白与动态血糖检验的结果与相关性分析

摘　　要：目的探讨早期糖尿病肾病诊断中尿微量白蛋白(UACR)与动态血糖监测(CGM)结果的相关性及其在早期诊断中的应用价值。方法选取2023年3月至2024年3月在厦门市思明区中华街道社区卫生服务中心门诊就诊的80例糖尿病患者作为研究对象,依据诊断结果分为肾损伤组40例,单纯糖尿病组40例,另外选取此时间段在本院门诊进行体检的健康人群40名作为对照组,所有研究对象均进行尿白蛋白/肌酐比值(UACR)检测和动态血糖监测。检测尿微量白蛋白水平,同时通过动态血糖监测系统记录患者的24 h血糖波动情况,分析UACR水平与动态血糖参数(平均血糖水平、血糖波动范围、血糖标准差)之间的相关性。结果UACR水平与动态血糖波动、平均血糖水平呈正相关(P<0.05)。结论早期糖尿病肾病患者的尿微量白蛋白水平与动态血糖波动有相关性。动态血糖监测结合UACR检测可以为糖尿病肾病的早期诊断提供依据。Objective Objective To investigate the correlation between urinary microalbumin(UACR)and continuous glucose monitoring(CGM)results in the diagnosis of early diabetic nephropathy(DN)and evaluate their clinical application value.Methods A total of 80 diabetic patients treated at Zhonghua Street Community Health Center,Siming District,Xiamen City,from March 2023 to March 2024 were enrolled and divided into two groups based on diagnostic results:a renal injury group(n=40)and a diabetes-only group(n=40).Additionally,40 healthy individuals undergoing routine physical examinations during the same period were included as controls.All participants underwent urinary albumin-to-creatinine ratio(UACR)testing and CGM.Urinary microalbumin levels were measured,and 24-hour glucose fluctuations(including mean glucose levels,glucose variability range,and standard deviation)were recorded via CGM.The correlation between UACR levels and dynamic glucose parameters was analyzed.Results UACR levels showed a positive correlation with glucose fluctuations and mean glucose levels(P<0.05).Conclusions Urinary microalbumin levels in early DN patients are associated with dynamic glucose fluctuations.Combined UACR and CGM may serve as a diagnostic basis for early-stage DN.
