产后康复治疗仪联合早期乳房按摩对产妇产后乳房胀痛及泌乳的影响

摘　　要：目的分析在产妇产后乳房疼痛、泌乳中应用治疗仪联合早期按摩乳房的应用效果。方法选取对象来自厦门大学附属中山医院,共96例,于本院就诊时间为2023年1月至2024年6月。经随机数表法分为对照(给予常规治疗)与观察组(给予康复治疗仪联合早期乳房按摩),比较两组疼痛程度(VAS评分)、泌乳乳动时间、催乳素水平、产后第3天泌乳量、睡眠质量[匹兹堡睡眠质量指数(PSQI)]及满意度。结果经干预,两组VAS评分均处于下降趋势,且观察组较对照组更低(P<0.05);经干预,观察组除去泌乳素水平、产后3 d泌乳量高出对照组外,泌乳乳动时间较对照组低(P<0.05);两组PSQI睡眠时间、睡眠连续性、睡眠深度、总分均较治疗前降低,且观察组睡眠时间、睡眠连续性、睡眠深度、总分较对照组低(P<0.05)。观察组本次干预满意度高出对照组(P<0.05)。结论按摩加仪器辅助治疗在产妇产后乳房胀痛及泌乳中应用效果显著,不仅可减轻疼痛,还可促进泌乳分泌,缩短泌乳乳动时间,同时提高睡眠质量。Objective Analyze the application effect of using a therapeutic device combined with early breast massage in postpartum breast pain and lactation in mothers.Methods The subjects were selected from Xiamen University Affiliated Zhongshan Hospital,totaling 96 cases.The inclusion period started from January 2023 and ended in June 2024.They were randomly divided into the control group(receiving conventional treatment)and the observation group(receiving rehabilitation treatment instrument combined with early breast massage).The pain degree(VAS score),time of milk ejection,prolactin level,milk volume on the third day postpartum,sleep quality[Pittsburgh Sleep Quality Index(PSQI)],and satisfaction were compared between the two groups.Results After the intervention,the VAS scores of both groups showed a downward trend,and the decrease in the observation group was higher than that in the control group(P<0.05).After the intervention,except for the prolactin level and milk volume on the third day postpartum,which were higher in the observation group than in the control group,the time of milk ejection in the observation group was lower than that in the control group(P<0.05).The PSQI sleep time,sleep continuity,sleep depth,and total score of both groups were lower than those before the treatment,and the sleep time,sleep continuity,sleep depth,and total score of the observation group were lower than those of the control group(P<0.05).The satisfaction of the observation group with this intervention was higher than that of the control group(P<0.05).Conclusions The application of massage combined with instrument-assisted treatment in postpartum breast distension and pain and lactation has a significant effect.It can not only relieve pain but also promote lactation secretion,shorten the time of milk ejection,and improve sleep quality.
