摘　　要：目的探讨膝关节骨性关节炎人工关节置换术后发生下肢静脉血栓的影响因素。方法收集2023年1月至2024年9月于首都医科大学附属北京天坛医院进行膝关节骨性关节炎人工关节置换手术治疗的118例患者的临床资料,根据静脉血栓发生情况将其分为血栓组(n=21)和非血栓组(n=97)。收集患者的一般资料及实验室指标,分析膝关节骨性关节炎人工关节置换术后发生下肢静脉血栓的影响因素。结果两组患者年龄、手术时间、止血带使用时间比较,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,年龄≥60岁、止血带使用时间≥90 min、手术时间≥90 min均是膝骨关节炎人工膝关节置换术后发生静脉血栓的独立危险因素(P﹤0.05)。结论年龄≥60岁、止血带使用时间≥90 min、手术时间≥90 min均是膝骨关节炎患者人工膝关节置换术后发生静脉血栓的危险因素,临床中应重点关注高龄患者,尽量缩短手术时间和止血带使用时间,以降低静脉血栓形成的发生风险。Objective To investigate the influencing factors of lower extremity vein thrombosis after arthroplasty for knee osteoarthritis.Method Clinical data of 118 patients with knee osteoarthritis who underwent artificial joint replacement surgery in Beijing Tiantan Hospital,Capital Medical University from January 2023 to September 2024 were collected and divided into thrombus group(n=21)and non-thrombus group(n=97)according to the occurrence of venous thrombosis.General data and laboratory indexes of patients were collected to analyze the influencing factors of lower extremity vein thrombosis after arthroplasty for knee osteoarthritis.Result There were significant differences in age,operation time and tourniquet use time between the two groups(P<0.05).Multivariate analysis showed that age≥60 years old,tourniquet use time≥90 min and operation time≥90 min were independent risk factors for vein thrombosis after knee arthroplasty(P<0.05).Conclusion Age≥60 years old,tourniquet use time≥90 min,and operation time≥90 min are all risk factors for venous thrombosis in patients with knee osteoarthritis after artificial knee replacement.Clinical attention should be paid to elderly patients,and the operation time and tourniquet use time should be shortened as much as possible to reduce the risk of venous thrombosis.
