下肢静脉曲张患者中非血栓性髂静脉压迫综合征的影响因素分析

摘　　要：目的探讨下肢静脉曲张(LEVV)患者中存在非血栓性髂静脉压迫综合征(NIVCS)的危险因素。方法收集2023年1月至2023年7月江苏省人民医院宿迁医院/宿迁市第一人民医院收治的145例LEVV患者的临床资料,根据NIVCS发生情况将其分为NIVCL组(n=30)和无NIVCL组(n=115),比较两组患者的临床相关指标,分析LEVV患者发生NIVCS的危险因素。结果两组患者性别、临床表现-病因学-解剖学-病理生理学(CEAP)分级、患肢侧别比较,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,CEAP分级C2级以上、左侧患肢均是LEVV患者发生NIVCS的危险因素(P﹤0.05)。结论CEAP分级C2级以上、左侧患肢均是LEVV患者中存在NIVCS的危险因素,临床上应及时发现高危患者,进一步行静脉造影确诊,并积极进行干预治疗。Objective To investigate the risk factors of non-thrombotic iliac vein compression syndrome(NIVCS)in patients with lower extremity varicose vein(LEVV).Method Clinical data of 145 patients with LEVV admitted to Jiangsu Province(Suqian)Hospital/Suqian First People's Hospital from January 2023 to July 2023 were collected.According to the occurrence of NIVCS,they were divided into NIVCL group(n=30)and non-NIVCL group(n=115),the clinical related indicators of patients in the two groups were compared.The risk factors of NIVCS in patients with LEVV were analyzed.Result There were significant differences in gender,clinical etiology anatomy pathophysiology(CEAP)grade and side of affected limb between the two groups(P<0.05).Multivariate analysis showed that clinical etiology anatomy pathophysiology(CEAP)grade C2 or above and left affected limb were all risk factors for NIVCS in LEVV patients(P<0.05).Conclusion CEAP grade C2 or above and left affected extremity are all risk factors for NIVCS in patients with LEVV.High-risk patients should be found in time,further venography should be performed to confirm the diagnosis,and active intervention treatment should be carried out.
