彩色多普勒超声对颅外颈动脉斑块性质和狭窄程度的诊断价值

摘　　要：目的探讨彩色多普勒超声对颅外颈动脉斑块性质和狭窄程度的诊断价值。方法收集2021年1月至2023年1月菏泽市立医院收治的97例颈动脉斑块患者的临床资料。记录所有患者就诊时的颅外颈动脉彩色多普勒超声资料,以数字减影血管造影(DSA)为金标准,分析彩色多普勒超声对颈动脉斑块性质和狭窄程度的诊断结果与金标准之间的一致性,运用受试者工作特征(ROC)曲线分析彩色多普勒超声对其诊断价值。结果彩色多普勒超声对颈动脉狭窄程度的诊断结果与金标准之间具有高度一致性(Kappa=0.822,P﹤0.05),对颈动脉斑块性质的诊断结果与金标准之间具有较高的一致性(Kappa=0.792,P﹤0.05)。ROC曲线分析结果显示,彩色多普勒超声对颈动脉狭窄程度、颈动脉斑块性质均具有中等诊断价值(AUC=0.778、0.838,P﹤0.05)。稳定斑块的RI、收缩期最大血流速度(PSV)、舒张末期血流速度(EDV)均明显低于易损斑块,差异均有统计学意义(P﹤0.01)。结论彩色多普勒超声对于颅外颈动脉斑块性质与颈动脉狭窄程度均具有较高的诊断价值,不同斑块性质在彩色多普勒超声下可见明显差异性表现。Objective To explore the diagnostic value of color Doppler ultrasound(CDFI)in the plaque nature and stenosis degree of extracranial carotid artery.Method The clinical data of 97 patients with carotid artery plaques admitted to Heze Municipal Hospital from January 2021 to January 2023 were collected.The extracranial carotid artery color Doppler ultrasound data from patients at the time of treatment were recorded,the digital subtraction angiography(DSA)was used as the gold standard,the consistency between the diagnostic results of color Doppler ultrasound for the nature of carotid artery plaques and degree of carotid artery stenosis and the gold standard was calculated,and the receiver operator characteristic(ROC)curve was used to analyze the diagnostic value of color Doppler ultrasound for the nature of carotid artery plaques and degree of carotid artery stenosis.Result The diagnostic results of color Doppler ultrasound for the degree of carotid artery stenosis were highly consistent with the gold standard(Kappa=0.822,P<0.05),and the diagnostic results for the nature of carotid artery plaques were highly consistent with the gold standard(Kappa=0.792,P<0.05).The ROC curve analysis results showed that color Doppler ultrasound has moderate diagnostic value for the degree of carotid artery stenosis and the nature of carotid artery plaques(AUC=0.778,0.838,P<0.05).The RI,peak systolic velocity(PSV)and end diastolic velocity(EDV)of stable plaques were lower than those of vulnerable plaques,the differences were statistically significant(P<0.05).Conclusion Color Doppler ultrasound has relatively high diagnostic value for the nature of extracranial carotid artery plaques and degree of extracranial carotid artery stenosis,and significant differences in plaque properties can be observed under color Doppler imaging.
